Every new zorgaanbieder under the Wkkgz reports to the CIBG in the three months before it starts, and may not treat a patient until it has. An instelling providing medisch specialistische zorg, or Zvw or Wlz care, also needs a toelatingsvergunning.
The meldplicht, for everyone
All new care providers report, solisten and zzp'ers included, and may not begin care before they have.1 The melding is an online questionnaire, best completed with eHerkenning at level 2+, and goes to the IGJ, with a bewijs van melding to download at once.1 Article 2 of the Wtza sets the duty in law.4 In 2025, 13,730 new providers reported.6
The toelatingsvergunning, for instellingen
Article 4 requires a vergunning from an instelling that provides medisch specialistische zorg, or care under the Wlz or the Zvw.4 Care by a huisarts, specialist ouderengeneeskunde, arts verstandelijk gehandicapten or verpleegkundig specialist is not medisch specialistische zorg.2 Since 1 January 2025 the duty also covers providers with fewer than 11 zorgverleners, and many already working received a vergunning van rechtswege.3

- Run the CIBG checks. The meldplichtcheck and the vergunningplichtcheck on toetredingzorgaanbieders.nl.
- Apply for the vergunning in good time. If you need one, the CIBG refuses it when it is not plausible that you meet the requirements.
- Prepare the people checks. The CIBG may ask for a VOG from the entity, owner, director or internal supervisor, and may run a Bibob check.
- Report in the three months before the start. Complete the online questionnaire and keep the bewijs van melding.
- Have the Wkkgz duties running. Klachtenregeling, klachtenfunctionaris, geschilleninstantie and the incident procedure, from day one.

The Wkkgz still applies in full once you start, set out in the guide to the quality system. Article 12 of the Wkkgz has the Minister keep a public register of every zorgaanbieder, with its addresses and the forms of care it gives.5
Questions
Can we start seeing patients before we report?
No. The Wtza says care may not begin before the melding, and the CIBG asks for it in the three months before the start.
Does a solo doctor need a toelatingsvergunning?
No. A solist or zzp'er in an eenmanszaak is outside the vergunningplicht, though every new provider still reports.
Who receives the melding?
The CIBG takes it through the online questionnaire and passes it to the IGJ, which uses it in its supervision.
