From the request to the signed note
CQC expects clinicians on home visits to carry the knowledge, skills and equipment for acute emergencies, chosen for the conditions they are likely to meet, and to take and record vital signs wherever the patient is.12
- Take the request. Record who is calling, for whom, the reason and how urgent it is, on the patient's own record.
- Request the visit. Agree a time with the patient or their family, and record consent to an AI scribe at the same moment.
- Prepare for the visit. Check the bag against the practice's list for the visit type, including any emergency medicines it carries.1
- Examine and record. Take and record vital signs, and offer a chaperone, with special consideration for examinations at home.23
- Sign the note before the next visit. The note, any letter and any referral are signed while the visit is fresh.
How wai handles it
- Appointments. Patients and families request visits from your own website at any hour, with a reminder before the doctor arrives.
- Evidence and audit trail. Consent to the scribe is recorded at booking, and flags for controlled drugs and safeguarding notes sit on the record at the point of care.
- Scribe. The note, letters and referrals are written from the visit, cited to guidelines such as NICE, with the transcript beside the note, for the doctor to read, edit and sign.
