For klinieken, zbc's and zorgaanbieders in the Netherlands, Inspectie Gezondheidszorg en Jeugd

IGJ readiness software, built to help you achieve an IGJ inspection with no measures.

The IGJ supervises every Dutch zorgaanbieder on the Wkkgz, the Wtza and the WGBO, reads patient files on its visits and publishes its reports and measures. wai runs booking, notes, patient messaging and payments for clinics, groups and hospitals, and keeps each as the evidence an inspector asks for, built to help you achieve an IGJ inspection with no measures.

Updated . Every IGJ fact is checked against igj.nl and the other official pages cited below.

What an IGJ inspector finds with wai

With wai, an inspector reads one file. The booking, the consent recorded, the signed note, every message and every complaint sit together, timed and named. A calamiteit review starts from the audit trail on the day it is found, and the file is kept for the twenty years the WGBO asks.

What does the IGJ hold a Dutch clinic to in 2026?

Five sets of duties, from three laws, each one supervised by the IGJ.

How does an IGJ inspector assess a clinic?

On site or from the desk, and always ending in a judgment and a report.2

  1. 01

    Inspectiebezoek

Announced or (partly) unannounced, 2,020 of them in 2025.

  2. 02

    Gesprekken

Interviews with professionals, the client council, the board and internal supervisors.

  3. 03

    Observatie

Inspectors may watch professionals at their daily work.

  4. 04

    Dossiers and documenten

Inspectors read documents and patient files, and article 24 of the Wkkgz extends their powers to dossiers.

  5. 05

    Desk-inspectie met oordeel

A judgment on written and digital information alone, 300 in 2025.

  6. 06

    Beoordelend gesprek

A conversation when risks are seen, to decide whether enforcement is needed.

How does an IGJ inspection end?

With a judgment and a published report, and when standards are missed, a measure that is often published too.3

  1. No measuresA judgment and a report, published on toezichtdocumenten.igj.nl
  2. Verscherpt toezichtExtra visits and improvement reports, 17 times in 2025
  3. AanwijzingA written instruction with a deadline, 26 in 2025
  4. BevelImmediate, valid seven days, 2 in 2025
  5. Last onder dwangsom or boete113 and 33 in 2025, a boete up to €33,500
  1. 2,020

inspection visits by the IGJ in 2025, beside 300 desk inspections with a judgment.4

  2. 1,790

calamiteiten reported to the IGJ in 2025, up from 1,540 in 2024.4

  3. 26

    aanwijzingen issued in 2025, against 21 in 2024 and 15 in 2023.4

  • After every inspection visit the IGJ gives a judgment and writes a report, published on its toezichtdocumenten site.
  • An inspection report stays online for 5 years after publication, and published measures stay visible for 3 years after they end.
  • A provider receives an openbaarmakingsbesluit first, and may object within 6 weeks.
  • A boete of up to €33,500 applies to a melding under article 11 that is not made.

What has changed for Dutch zorgaanbieders?

Three things a clinic should know in October 2026, each marked as in force or scheduled.

  1. In force. In 2025 the IGJ stopped asking hospitals for a calamiteitenrapportage, and asks for bestuurlijke reflectie on the improvement measures instead.
  2. In force. Since 1 January 2025 the Wtza vergunningplicht also covers providers with fewer than 11 zorgverleners giving Zvw or Wlz care, and many already working received a vergunning van rechtswege.5
  3. Scheduled. An amendment to article 3a of the Wkkgz, on the availability of acute care, takes effect on 1 January 2027.

Who the Wtza and the Wkkgz apply to

Every zorgaanbieder answers to the Wkkgz. The Wtza decides whether it only reports or also needs a vergunning.

Proposed, and not in force

One bill a Dutch clinic will hear about, stated as it stands.

  1. The Wet integere bedrijfsvoering zorg- en jeugdhulpaanbieders (Wibz, 36.686) is a bill, and nothing in it applies to a clinic today.6
  2. On 3 June 2026 the government wrote to parliament about the date for sending its reconsideration of the bill.

How do you prepare for an IGJ inspection?

A readiness checklist in four parts.

01Toetreding and the register

  • Bewijs van melding from the CIBG on file for every legal entity.
  • A toelatingsvergunning, or the vergunning van rechtswege, wherever the Wtza requires one.
  • The register of zorgaanbieders checked for every location and form of care.
  • eHerkenning at level 2+ for the person who makes the CIBG melding.

02Kwaliteit and meldingen

  • A written internal procedure for incidents, known to every member of staff.
  • A calamiteit procedure with the 3 working day and 8 week clocks written in.
  • Quality data collected, checked and acted on, as article 7 asks.
  • Written agreements with every zorgverlener who is not an employee.

03Klachten and geschillen

  • A written klachtenregeling, made known to patients.
  • A named klachtenfunctionaris, independent in the role and at no cost to the patient.
  • Every complaint answered in writing within six weeks, or ten with notice.
  • Membership of a recognised geschilleninstantie, shown to patients.

04The dossier

  • Ten files an inspector could read cold, complete, signed and timed.
  • Consent and the information given recorded in the file.
  • Retention set to twenty years from the last change.
  • Access logged under NEN 7513, and requests for inzage en afschrift answered.

What will an IGJ inspector ask you?

Questions to expect, what to show, and where wai holds the answer.

01May I see the files of these patients?

What to show

Complete dossiers, consent included, that match what the clinician describes.

Where wai has it ready

The note, consent, letters and messages sit on one record, signed and timed.

02Show me how you report and investigate a calamiteit.

What to show

The procedure, the meldingen made, the investigation reports and the measures taken.

Where wai has it ready

The audit trail and the signed note give the investigation its timeline, to the minute.

03How does a patient complain here, and who helps them?

What to show

The klachtenregeling, the klachtenfunctionaris, the answers sent and the geschilleninstantie.

Where wai has it ready

Every message sits in one thread with response times, and reports run for any period.

04How do you know the quality of your care?

What to show

The data collected, how it is reviewed, and what changed as a result.

Where wai has it ready

Reports on response times, the front desk and takings, produced the day they are asked for.

05Who can open a patient file, and how is that logged?

What to show

Roles, access controls, an access log and where records are hosted.

Where wai has it ready

Role-based access enforced in the database, every read logged, records hosted in the clinic's own country.

06How do you use generative AI, and who checks it?

What to show

Where it is used, who checks the output and how its risks are assessed, as the IGJ asked in February 2025.

Where wai has it ready

AI drafts the note and the reply. The clinician signs, and a person approves every message.

What is at stake when the IGJ visits?

Reports and measures are public, a missed melding carries a boete, and the IGJ now sees more new providers than ever.

  1. 13,730

new zorgaanbieders reported to the CIBG in 2025, 2,170 of them instellingen that need a vergunning.4

  2. €33,500

is the highest bestuurlijke boete under the Wkkgz, for a calamiteit or other melding that is not made.11

  3. 113

lasten onder dwangsom and 33 boetes were imposed by the IGJ in 2025.4

Questions

What is the best software for IGJ compliance?

The best IGJ compliance software is the system your clinic already runs on, so the evidence is a by-product of the care. Look for one record behind booking, notes, patient messaging, payments and access, a full audit trail, AI output that a person signs, and records hosted under your country's rules. wai is built that way, to help you achieve an IGJ inspection with no measures.

Is there IGJ approved software?

The Inspectie Gezondheidszorg en Jeugd supervises zorgaanbieders on the Wkkgz, the Wtza and the WGBO. It does not approve, certify or endorse software, wai included, and software cannot earn an inspection judgment on a provider's behalf. wai keeps the records your team uses to show IGJ how care is delivered, and IGJ decides the inspection judgment.

Does the Wkkgz apply to a private or cosmetic clinic?

Yes. The Wkkgz applies to every zorgaanbieder, institutions and self-employed professionals alike, and Rijksoverheid names cosmetic treatment and alternative care among them. It does not apply to support under the Wmo or to jeugdhulp.

Does a new clinic need a Wtza toelatingsvergunning?

An instelling providing medisch specialistische zorg, or care under the Zvw or the Wlz, needs one. Since 1 January 2025 that includes providers with fewer than 11 zorgverleners. A solist in an eenmanszaak only reports, and every new provider reports before the first patient.

How quickly must a calamiteit be reported?

Without delay, and within 3 working days of establishing that it is a calamiteit. If that is unclear, the provider has up to 6 weeks to find out, and when the IGJ asks for a report it allows 8 weeks.

How long must a Dutch clinic keep a patient file?

Twenty years from the last change to the file, or longer where good care requires it, under article 7:454 of the Burgerlijk Wetboek. A patient may ask for the file to be destroyed, within the limits of article 7:455.

Does wai work in Dutch?

Yes. wai drafts replies in the language the patient writes in, Dutch, English or any other, for your team to approve.

Where does wai keep our records?

In its own country, under that country's rules, encrypted, with a full audit log. We set this up with you before go-live.

See wai on your own diary, notes and inbox.

Fifteen minutes on video, opening one patient file the way an IGJ inspector would. Built to help you achieve an IGJ inspection with no measures. wai hosts each clinic's records in its own country, under that country's rules. We set this up with you before go-live.
