What quality system does the Wkkgz require of a Dutch clinic?

The Wkkgz asks for good care, organised so that it can be delivered, and a system that collects quality data, tests the care against it and changes what does not work. Article 9 adds a written internal procedure for incidents that staff know and use.

Five articles that make the system

ArticleWhat it asks
2Goede zorg, safe, effective, efficient and client-centred, respecting the patient's rights
3Care organised with the people, materials and clear responsibilities it needs
4An instelling checks how its zorgverleners have worked before, and binds non-employees by written agreement
7Systematic collection of quality data, testing against it, and change where needed
9A written internal procedure for incidents, with the personal data seen only by the people who handle them

Make it a system that runs

Article 7 asks the zorgaanbieder to collect and register quality data so it can be compared with providers of the same kind, to test whether the care is good against that data, and to change the way care is organised where it is not.1 Rijksoverheid adds that staff must be able to report incidents safely, and leaves the provider to choose how.3 Article 11.0a makes any clause that stops someone sharing information about an incident void.1

  1. Choose a few measures that matter. Response times, complaints, incidents, re-treatments and patient experience, measured the same way every month.
  2. Collect them from the work itself. Data taken from the system the clinic runs on is complete and needs no second entry.
  3. Review them on a fixed date. A monthly quality meeting with a named lead, and minutes that record the decisions.
  4. Write the incident procedure. How to report, who handles it, and who may see the personal data, as article 9 requires.
  5. Check every zorgverlener. How they have worked before, and a written agreement for anyone who is not an employee.
  6. Govern generative AI. The IGJ asks providers to know its risks and limits, check its results and assess risks at purchase, implementation and use.

Article 10 also asks the clinic to tell a patient, on request, about its prices, quality and patient experience, and about waiting times.1 On AI, the IGJ wrote in February 2025 that providers need enough knowledge of its risks and limits and must check its results.2 For incidents that become calamiteiten, read the guide to reporting a calamiteit.

Questions

Does the Wkkgz prescribe a quality certificate?

No. Article 7 sets what the system must do, collecting data, testing the care and changing it, and names no certificate.

Must a small clinic have an incident procedure?

Yes. Article 9 requires a written internal procedure from every zorgaanbieder, and the provider chooses how to organise it.

What does the IGJ expect of AI in care?

Enough knowledge of its risks and limits, a check on its results, and attention at purchase, implementation and use, with medical device rules where they apply.
