Report a calamiteit to the IGJ without delay, within 3 working days of establishing that it is one, through the meldformulier on mijn.igj.nl. When the IGJ asks for an investigation report, it allows 8 weeks.

What counts as a calamiteit

A calamiteit is an unintended or unexpected event, related to the quality of care, that led to a patient's death or serious harm.1 Article 11 of the Wkkgz requires the zorgaanbieder to report it to the inspectorate without delay, beside violence in the care relationship and the end of a contract with a zorgverlener for seriously deficient performance.2 Each of the three has its own meldformulier on mijn.igj.nl.3

Two clocks, then the report

  1. Secure the patient and the facts. Care first, then preserve the record as it stood.
  2. Tell the patient. Article 10 lid 3 asks you to tell the patient or their family the nature and cause of the incident without delay, and note it in the dossier.
  3. Decide whether it is a calamiteit. If it is unclear, you have up to 6 weeks from noticing the event to find out.
  4. Report within 3 working days. Once established, use the form Verplichte melding calamiteit on mijn.igj.nl.
  5. Investigate within 8 weeks. When the IGJ asks for a report, ask meldpunt@igj.nl for more time before the 8 weeks end if you need it, with a reason and a new date.
  6. Act on the findings. The IGJ closes the matter when there is no longer a serious threat, the investigation was sound and the measures are taken.

The IGJ answers within 6 weeks of receiving the report, and may run its own investigation.1 It received 1,790 calamiteit reports in 2025, 750 of them from medisch specialistische zorg, and hospitals now send a bestuurlijke reflectie instead of a full calamiteitenrapportage.4 A melding that is not made can bring a bestuurlijke boete of up to €33,500.2

Questions

Where do we report a calamiteit?

To the IGJ, using the meldformulier Verplichte melding calamiteit on mijn.igj.nl.

Do we always send the full investigation report?

No. The IGJ says the full report is not always needed, and may ask for documents such as a summary instead.

What else must be reported under article 11?

Violence in the care relationship, and ending a contract with a zorgverlener for seriously deficient performance, each without delay.
