How do you organise the evidence for a MOPH inspection?

File it by the five areas MOPH regulates, the facility licence, inspection, practitioners, patient safety and patients' rights, and make each item come from the system the work happens in. Once Fitness to Practice opens a complaint, it can obtain the patient's whole file, so the file must already be complete.

Why the file has to be whole already

When Fitness to Practice begins its preliminary assessment of a complaint, that counts as an official delegation to obtain a copy of the patient's file and to contact every facility where the patient was treated.1 The section also audits practitioners' performance and makes inspection visits of its own.2 The Bill of Rights requires the patient's consent or refusal to be in the record.3

MOPH areaEvidenceWhere it should come from
Facility licenceThe licence, the approved layout, current FLI dataThe licensing file
InspectionComplete patient files, signed and timedThe clinical record
PractitionersLicences, scopes, who did whatAccounts, roles and the audit trail
Patient safetyIncident facts, classification, actionsThe audit trail and the incident review
Patients' rightsBill displayed, consent, complaints and repliesThe record and the patient's message thread

Use MOPH's own structure for incidents

The Qatar Patient Safety Classification, launched on 11 January 2026, sorts each event on four levels, its domain, its severity and impact, its contributory factors, and the corrective and preventive actions taken.4 Filing incident evidence the same way means the facility's own review and a national report describe an event in the same terms. The Health Facilities Department's field inspections then read the same files.5

  1. Make five folders. One for each area MOPH regulates, each with a named owner.
  2. Point to the source. For each item, write where it lives in the live system, so nobody prints and copies records.
  3. Test one file a week. Ask for a random patient's file and time how long it takes to open whole.
  4. File incidents on four levels. Domain, severity, contributory factors and actions, as the national classification does.
  5. Keep complaints with replies. Every complaint beside the reply and its date, so the outcome is visible.

Questions

Can MOPH obtain a patient's file?

Yes. Once Fitness to Practice begins its preliminary assessment of a complaint, it may obtain a copy of the patient's file and contact every facility that treated them.

Should we print evidence for an inspection?

Better to produce it from the system the work happens in, so it is current and shows who did what and when.

How should incidents be filed?

On the four levels of the Qatar Patient Safety Classification, domain, severity and impact, contributory factors, and corrective and preventive actions.
