What happens during a Joint Commission ambulatory survey in 2026?

Most Joint Commission surveys are unannounced and come 30 to 36 months after the last full survey, and most survey time is spent on tracers that trace real patients through your care. The surveyors post a preliminary Summary of Survey Findings at the exit conference, and the accreditation decision comes only after the post-survey work.

When the survey comes

Organisations can expect an unannounced survey between 30 and 36 months after their previous full survey.1 Initial surveys that are not for deemed status come with 30 days' notice. Some ambulatory resurveys, including office-based surgery, sleep centres, telehealth and ASCs not using accreditation for deemed status, come with seven business days' notice.2 Accreditation is valid for about three years once awarded.1

The order of the survey

ActivityWhat happens
Arrival and preliminary planningThe first hour plans the survey from your documents, and includes a safety briefing of five minutes or less.
Opening conference and orientationIntroductions, the agenda and how the organisation runs.
Individual tracersMost of the survey. Blocks of 60 to 120 minutes at the point of care.
Competence and credentialingOrientation, competency and how privileges are granted, checked against files.
Data management system tracerHow data is collected, checked and used for improvement.
Environment of care and emergency managementPlans, risks and, for ASCs, the Life Safety Code assessment.
Leadership sessionHow leaders set priorities and act on data.
CEO exit briefing and exit conferenceThe Summary of Survey Findings, the SAFER Matrix and next steps.

This order comes from Joint Commission's ambulatory Survey Activity Guide, September 2026 release.2 An ASC on a deemed status survey also has a Life Safety Code surveyor, and surveyors observe at least one surgical procedure.2

How tracers work

Tracers use the organisation's own information to follow the care of a number of patients through the whole care process, and identify issues in a step or at the interface between steps.3 There are individual tracers, system tracers and program-specific tracers.3 The guide to the questions surveyors ask staff in a tracer lists them.

  1. Write the arrival plan. Who greets the surveyors, who checks the photo ID and Joint Commission Connect, who is the contact person, and where the surveyors base themselves.
  2. Name participants for each session. Fill the activity list with names, phone numbers and back-ups for every session.
  3. Keep the document list current. Update the documents monthly so they are ready on the first morning.
  4. Prepare the safety briefing. Five minutes on fire, emergencies, workplace violence and anything current.
  5. Run mock tracers. Trace patients who move between services, recent admissions and recent discharges, as Joint Commission's tips suggest.
  6. Plan the exit. Decide who attends the exit conference and who reads the report on Joint Commission Connect.

After the exit conference

The Summary of Survey Findings report given at the exit conference does not contain an accreditation decision.2 Findings are plotted on the SAFER Matrix by likelihood of harm and scope.4 The decision is made after the post-survey activities, and can be Accreditation, Accreditation with Follow-up Survey, Limited Temporary Accreditation, Preliminary Denial of Accreditation or Denial of Accreditation.5 Next, read the guide to Evidence of Standards Compliance.

Questions

Is the Joint Commission survey announced?

Most are unannounced. Initial surveys that are not for deemed status get 30 days' notice, and some ambulatory resurvey types get seven business days' notice.

How long does a Joint Commission ambulatory survey last?

It depends on the size and services in your application. Joint Commission's sample agendas run from one surveyor for two days to several surveyors for three days, and a typical office-based surgery survey lasts one day.

Do surveyors give the accreditation decision at the exit conference?

No, the exit conference reviews preliminary findings, and the decision is made after the post-survey activities, including any Evidence of Standards Compliance.
