What belongs on a Joint Commission ambulatory care standards checklist?

Build the checklist from four things Joint Commission publishes, the 16 chapters of the 2026 Comprehensive Accreditation Manual for Ambulatory Care, the National Patient Safety Goals for ambulatory care, the survey document list and the post-survey steps. Check each item against the live record.

The chapters the standards sit in

The Comprehensive Accreditation Manual for Ambulatory Care, effective January 1, 2026, groups its accreditation requirements into 16 chapters, from Accreditation Participation Requirements to Waived Testing.1 The full standards and elements of performance are in E-dition for accredited organisations, and Joint Commission publishes a public, searchable version for every program.2 The Joint Commission hub lists the chapters with their codes.

The documents surveyors ask for on arrival

The ambulatory Survey Activity Guide lists what must be ready when the surveyors' identity is validated. It includes 12 months of performance and quality improvement data, 12 months of infection control surveillance data and the infection control plan, environment of care plans and evaluations, the organisation chart, the list of sites and locations, the day's appointment or surgery schedules, contracted services and the most recent culture of safety evaluation.3 If the documents are not available, surveyors begin with an individual tracer.3 For a deemed ASC, add the list of surgeries from the past six months and any transfers or deaths in the past 12 months.3

  1. Assemble the document list. Put every item from the Survey Activity Guide's document list in one place, with an owner and a date.
  2. Walk the safety goals. Observe two identifiers, medication labelling, hand hygiene and a time-out during real care.
  3. Trace three patients. Choose patients from high-risk services and trace each from first contact to the end of the episode, using the record alone.
  4. Test credentials files. Pick three clinicians and confirm licence, training, privileges and reappraisal are current and findable.
  5. Test the data. Ask the person who owns each measure to explain how it is collected, checked and used.
  6. Write findings as RFIs. Score each gap by likelihood of harm and scope, as the SAFER Matrix does, and fix the high-risk ones first.

The checklist after the survey

Each finding that needs it must have Evidence of Standards Compliance submitted within 60 days, with the date the actions were completed and how compliance will be sustained.4 Practices in New York City and Miami also check state rules for office-based surgery, covered in the office-based surgery accreditation guide.
