Summary
Los Angeles clinics run AI for notes and patient messages under HIPAA and California law. AI may never present itself as a licensed clinician, clinical messages need a disclaimer unless a licensed clinician reviews them, recording needs every party's consent, and a negligent release of medical information carries statutory damages. With wai the team approves each draft and the physician signs clinical content.
A Los Angeles clinic on wai answers a Spanish WhatsApp at 9pm and a Korean enquiry by morning, and its diary fills. California's newest AI rule, AB 489, took effect this year, and it sits beside the rules below.

The private care market in Los Angeles
The Medical Board of California counts 33,778 physicians with a renewed, current license and a Los Angeles County address, 26% of the 127,714 with a California address and the largest of any county.1 Neighboring Orange County adds 11,795.1
The county's private market runs from family practices in the San Gabriel Valley to aesthetic and concierge clinics on the Westside.
The rules a Los Angeles clinic works under
Physicians here are licensed by the Medical Board of California.1 HIPAA applies as it does across the US, and the HIPAA and AI scribes guide covers it.
Business and Professions Code 4999.9, added by AB 489 and in force since 1 January 2026, prohibits any term in an AI system's advertising or functionality that implies the care or advice is coming from a licensed natural person.2 Each use is a separate violation, enforced by the relevant licensing board.2 Give any assistant a plain name with no clinical title, and keep clinical answers with the clinician.
Business and Professions Code 2400 gives corporations and other artificial legal entities no professional rights, privileges or powers.3 For the aesthetic and wellness clinics common in Los Angeles, that keeps clinical judgment with the licensed physician, whoever owns the business. AI drafts, and the physician decides and signs.
AB 3030's disclaimer on AI messages about clinical information falls away once a licensed clinician has read and reviewed the message.6 In wai, every clinical message waits for that review before it sends.
California is an all-party state for recording. Under Penal Code 632, everyone in a confidential conversation must agree before it is recorded.4
Under Civil Code 56.36, a patient can sue for $1,000 in nominal damages over a negligent release of medical information without proving any loss, and negligent disclosure also carries fines up to $2,500 per violation.5
wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. wai is compliant with these rules and built for clinics in Los Angeles.
The languages Los Angeles patients write in
56.6% of Los Angeles city residents aged five and over speak a language other than English at home, and 55.2% across the county, according to the 2024 American Community Survey.7 Spanish is spoken at home by 1,440,657 city residents, 39.0%, and 46.9% of them speak English less than very well.7
Tagalog reaches 2.3% of the city and Korean 2.2%, with 62.3% of Korean speakers reporting English less than very well.7 wai reads messages in any language, and a team member approves each reply before it goes.8
What a demo shows in Los Angeles
The demo opens on a Korean enquiry sent at 9.15pm from Koreatown, asking about a first consultation this week. wai reads it in the patient's thread, reads the clinicians' busy times from Google Calendar or Outlook and drafts a reply in the patient's language offering two open times. Next morning the coordinator approves it in one tap. The patient books on the clinic's website and the deposit lands in the clinic's own Stripe account, in US dollars.
Then a Spanish WhatsApp from Boyle Heights asks to move an appointment, and a missed call from Santa Monica is answered by a drafted text. Both wait for one tap from the front desk. After the consult, the physician reviews and signs the drafted note and patient letter.
For health systems and payers in Los Angeles
Hospital groups and health plans with large Spanish- and Korean-speaking memberships can start with one care pathway built on wai, under their own brand. The organisations page sets out how a build starts.
Where to start
First, audit every place an AI touches patients, from the booking page to the chat greeting, and remove any title or phrase that implies a licensed person, as section 4999.9 requires.2 Second, name the licensed clinician who reviews each type of clinical message.6
Third, put the recording consent question in the intake questionnaire, starting from the consent form template.4 The law here is as read on 14 October 2026, and the clinic's counsel should confirm how it applies to the practice.
Send us last month's enquiries by language, and we show the evenings they would have filled.
Frequently asked questions
Can an AI receptionist in California call itself a nurse or doctor?
No. Since 1 January 2026, Business and Professions Code 4999.9 prohibits AI wording that implies care or advice comes from a licensed person, and each use is a separate violation.2
Does AB 489 reach the booking page and the chat greeting?
Yes. Section 4999.9 covers terms used in an AI system's advertising or functionality, so every channel where an assistant speaks to patients counts.2
What can a negligent release of medical information cost?
A patient can recover $1,000 in nominal damages without proving any loss, and negligent disclosure carries fines up to $2,500 per violation.5
Can an aesthetic clinic owned by a non-physician use an AI scribe?
Clinical judgment must stay with the licensed physician, since Business and Professions Code 2400 gives corporations no professional rights.3 The physician reviews and signs every AI-drafted note, and the clinic's counsel should confirm the ownership structure.
Which languages do Los Angeles patients write in most?
Spanish leads at 39.0% of city residents aged five and over, then Tagalog at 2.3% and Korean at 2.2%.7 Each draft reply is approved by the team before it is sent.
