  • A medical site lead for every Zurich location.
  • Every clinician's cantonal licence on file, and every arrival and departure notified.
  • Name, first name and role worn visibly by every professional in a Geneva institution.

3Records

  • Every entry shows its author, and electronic entries are dated and unalterable.
  • Records kept at least 10 years, with Zurich's Merkblatt recommending 20.
  • Patients can see their record and receive copies.
  • A plan for the records if the clinic or a doctor stops practising.

4Quality

  • Procedures written down and kept current.
  • Incidents reported in writing, with corrective action.
  • A complaint process patients can find.
  • For basic insurance billing, an internal reporting and learning system.

5What a Swiss cantonal licensing inspector may ask

  • Which professionals work here, and are they licensed? What to show, an up to date staff list with each person's cantonal licence.
  • How are incidents recorded and acted on? What to show, written incident reports, with corrective and preventive measures.

Questions

Has Swiss cantonal licensing approved this swiss cantonal licensing inspection checklist?

No. Swiss cantonal licensing does not approve templates or software. This template is built from Swiss cantonal licensing's own published standards and our guides, and your clinic adapts it, signs it off and owns it.

Can we use it as it is?

Use it as a starting point. Replace every part in square brackets with your clinic's own details, check it against your services, and have your clinical lead sign it off before it is used.

How do we get the editable version?

Enter your work email on this page and we send the editable document. The full template is on this page to read before you ask.
