HAS certification visit checklist
1Engagement and scope
- Calista accounts current, with an administrator named by the legal representative.
- Every site and activity declared, since an undeclared activity found at the visit suspends it and is reported to the ARS.
- ARS authorisations current for every activité de soins and heavy equipment.
- Visit dates and the expert team checked within two weeks of notification.
2The 21 impératifs
- Each impératif criterion evaluated internally with the HAS grids.
- Adverse events declared, analysed and fed back to teams and user representatives.
- e-Satis email collection tracked, and results shared with each unit every six months.
- A named owner for every gap, with a date.
3Patient records
- Ten records a patient traceur could read cold, complete, signed and timed.
- Consent and the personne de confiance recorded.
- A discharge letter given to every patient.
- Requests for record access answered within 8 days.
4Patients and users
- The CDU in place, informed of every complaint and every serious adverse event.
- Patients told they can give feedback, complain, praise or declare an adverse event.
- The certification macaron and quality indicators displayed at reception and on the website.
- The user representatives briefed and invited to the opening meeting.
5What an HAS inspector may ask
- May I meet this patient, and then the team that cared for them? What to show, a consenting patient, the team, and a record that matches what both describe.
- Show me how adverse events are declared and analysed here. What to show, declarations on the national portal, volet 2 within three months, and the feedback to teams.
- How do patients tell you about their experience? What to show, e-Satis participation, internal surveys, complaints and praise, and what changed as a result.
- How was this patient informed, and in which language? What to show, what the patient was told, when they agreed, and where it is written.
- Who can access patient data, and how is it secured? What to show, roles, access controls, an access log and where records are hosted.
- How do you use AI in your organisation? What to show, where it is used, who checks the output, and how governance oversees it, as criterion 3.4-06 asks.
