How do you renew an autorización de funcionamiento in Madrid?

A Madrid operating authorisation runs eight years, four for assisted reproduction, and the renewal must be requested at least three months before it ends or the authorisation lapses. Consultas and similar centres without fixed electromedical equipment renew by declaración responsable, and the rest file a full renewal that is treated as granted if unanswered after three months.

What Decreto 205/2023 changed

Decreto 205/2023 rewrote artículo 13 of Decreto 51/2006. The operating authorisation and its entry in the Registro de Centros, Servicios y Establecimientos Sanitarios are renewed every eight years, every four for centres that carry out assisted reproduction, and the renewal is requested at least three months before the end.¹ A renewal not requested in time lets the authorisation lapse, and the centre must apply again from the start.¹ The Consejería decides within three months, and if it has not answered by then the renewal is treated as granted.¹³

Consultas médicas, consultas of other health professionals, polyclinics, centros de reconocimiento, mental health centres and other specialised centres without fixed electromedical equipment or assisted reproduction only file a declaración responsable.¹ In it the holder states that the centre meets the decree's requirements, that it holds the supporting documents and will produce them when asked, and that it will keep meeting them for the next eight years.¹²

A renewal you can sign with confidence

1. Diary the date. Eight years from the authorisation or the last renewal, with the filing deadline three months earlier.
2. Check which route applies. Declaración responsable for consultas and similar centres without fixed equipment, a full renewal for the rest.
3. Reconcile the staff list. Every professional on the plantilla with their degree, colegiación and insurance, and every change already notified.
4. Compare the care offer. The services the centre provides today against the oferta asistencial it is authorised for.
5. Pay the fee and file. Through the Cuenta Digital, with the payment reference on the form.
6. Keep the evidence ready. A declaración responsable means the documents are produced when the Comunidad asks, at any time.

Between renewals, the holder notifies every change to the health staff, joiners and leavers alike, with no fee, and keeps the current staff list at the centre for inspection.⁵ Staff lists and staff identification are displayed under artículo 21 of Decreto 51/2006.⁴ Ley 14/1986 lets inspectors enter any health centre at any time without notice.⁶ The fee for the renewal itself is the operating fee, 96.21 euros for a consulta without X-ray.³
