How does JCI accreditation work, step by step?

Learn the standards, form a team, close the gaps, apply 6 to 12 months ahead, host the survey, act on the findings, then keep the standard for three years until reaccreditation. JCI publishes that path on its own process page.

The path JCI describes

  1. Learn. Read the standards manual and the survey process guide for your programme.1
  2. Form an accreditation team. Leaders and owners for each chapter, with the time to do the work.
  3. Prepare. A gap analysis, an action plan and mock tracers on real patients.
  4. Apply. Submit the application on JCI Direct Connect, at least 6 to 12 months before you want the survey.1
  5. The on-site survey. Surveyors evaluate more than a thousand measurable elements, with a six-month look-back.1
  6. Findings. Findings arrive on the SAFER Matrix, placed by likelihood to harm and scope.2
  7. Decision and improvement. The accreditation decision, and a Strategic Improvement Plan where JCI asks for one.
  8. Between surveys. Keep compliance continuous and reaccredit every three years.3

What the survey day looks like

JCI's agenda includes an opening conference, leadership interviews, a review of staff qualifications, a facility tour and a closing leadership conference.1 Most of the time is spent in tracers, where surveyors follow a patient's care through the hospital and talk to the staff who gave it.

After accreditation

Hospitals and academic medical centres now have five Continuous Engagement touchpoints between triennial surveys, virtual and led by a JCI survey expert, and the next survey is shaped by previous findings.4 Accreditation runs on a three-year cycle.
