How do you organise evidence for Qmentum and provincial accreditation?

Map each standard to the record that proves it, and keep that record where the work happens. Qmentum Global ends its four-year cycle with a short-notice on-site assessment, so the evidence has to exist on an ordinary day.

The Qmentum Global cycle

ActivityWhat it isWhat to keep ready
Self-AssessmentCustomizable self-assessments to find strengths and areas to improveYour rating for each criterion, and the record behind it
Survey instrumentsThe HSO Global Workforce Survey and the HSO Governing Body AssessmentResults, and what leaders changed because of them
AttestationA self-directed assessment of attestable criteria before the on-site visitThe evidence for every criterion you attest to
On-Site AssessmentA short-notice assessment by third-party surveyorsRecords your team can open in minutes
Quality Improvement Action PlanA digital tool to turn assessment results into action itemsOwners, dates and the result of each action

Qmentum Global for Canadian Accreditation moves from 38 Required Organizational Practices to 24 Required Safety Practices.1 The cycle runs over four years, with self-assessment and survey instruments first and the on-site assessment last.2

How provincial assessors sample and score

Each ICHSC requirement comes with What To Look For guidance, usually phrased as a question, which may name the documents and records required. Assessors sample by judgement, from high-volume, complex or error-prone services and from past non-conformances, and keep sampling until they are confident.3 The CPSBC rates each criterion Yes, No or Not Applicable, every mandatory criterion must be Yes for full accreditation, and only a No appears in the report.4

  1. List every criterion that applies. From the standards for your programme, and mark which are mandatory.
  2. Name the record that proves each one. A signed note, a consent, a minute, a report or a log, never a slide.
  3. Keep the record where the work happens. Evidence made after the fact is weaker than evidence made during care.
  4. Test it on high-risk services. Sample your own busiest and most complex services, as an assessor would.
  5. Track actions to closure. Every finding gets an owner, a date and the record that shows it closed.

Questions

How long is the Qmentum Global cycle?

Four years, with self-assessment, survey instruments and attestation before a short-notice on-site assessment.

What replaced Required Organizational Practices?

Required Safety Practices. Accreditation Canada streamlined 38 ROPs into 24 RSPs.

Do CPSBC best practice criteria count toward full accreditation?

No. Every mandatory criterion must be met, and a best practice criterion rated No is noted as an opportunity for improvement.
