Key points

Summary

Choose one system where every practitioner writes into the same patient record, every referral between disciplines is booked before the patient leaves, and access is set by role. Judge it on the internal referral and the shared diary, because that is where a mixed clinic keeps its patients.

A GP, a psychologist, a dietitian and a physiotherapist build one plan around one person, and the patient leaves with every appointment in it booked. With the record and the diary shared, a referral down the corridor becomes a booking in the same visit.

The shared record decides the choice

The physiotherapist needs the GP's imaging request, the dietitian needs the latest bloods, and the psychologist needs what the rest of the team already knows, within what the patient has agreed to share.

WHO described interprofessional collaboration in 2010 as one of the most promising answers to the global health workforce gap.3 On one record, each practitioner writes in a format suited to their profession, and every note, letter, result and message sits on the same patient timeline, in the order it happened.

The internal referral is the product

The referral from one colleague to another keeps a patient inside the clinic when it becomes a booking.

A study of primary care referrals to 20 high-volume specialties in a large US health system, between July 2015 and June 2016, found that only 34.8% of 103,737 referral scheduling attempts resulted in a documented completed appointment.1 In 38.9% of attempts no appointment date was recorded at all.1 Those referrals crossed a large network, and the lesson for a clinic is the mechanism. Booking the referral in the room carries the patient to the next appointment.

Refer from inside the record, see the receiving practitioner's real availability, hold a slot, and send the patient one confirmation with any payment or intake form attached. The referring clinician then sees the appointment, and later the note, on the same record.

One diary across practitioners

Appointment lengths differ by discipline, and many patients want two appointments back to back on the same morning.

Look for every practitioner's availability in one view, with booking across disciplines in one step. Booking should read each practitioner's busy times from Google Calendar, Outlook or Apple Calendar so patients are only offered open times. Prices and deposit terms are set per service, so a 20-minute medical review and a 60-minute psychology session each carry their own.

One combined reminder for a morning of appointments reads well, and a cancellation in one discipline is a slot to offer to the waitlist.

Sharing for care, access by role

Professions need different parts of a shared record, and some notes, such as a psychologist's session notes, belong to a narrower circle.

In the United States, HIPAA's minimum necessary standard does not apply to disclosures to or requests by a health care provider for treatment.4 For uses inside the clinic, a covered entity must identify which people or classes of people in its workforce need access to protected health information, and which categories of it, and make reasonable efforts to limit access to match.5 The same design, sharing for care and access set by role, fits other countries' rules too.

Set a role for each discipline, restrict sensitive note types, and keep an audit log of who opened what. Ask to see these settings working in the demo, signed in as each role.

How wai runs a multidisciplinary clinic

wai keeps the consult and the front desk on one record, so a patient seen by one practitioner is known to the next.6 wai Clinicians drafts the note during the consult and the letter after it, and the clinician reviews and signs both.6

wai Admin keeps email, web, WhatsApp and calls in one thread per patient, and checks availability across the clinic's practitioners and any partner providers when it books.7 Replies are drafted in the clinic's voice and nothing operational sends without the team's approval.8 Access is role-based and least-privilege, enforced in the database, and every message, draft, approval and signature is written to an event log the clinic can export.8

More on running the whole clinic on one platform, and on what a good letter between colleagues contains in the referral letter.

Where to start

List the five most common internal referrals in the clinic, for example GP to physio or GP to dietitian, and count how many from the last quarter became a completed appointment. That is your baseline.

Bring three real patient pathways to every demo and run them live, from first enquiry to the second discipline's signed note. Score each system against the table below, test role-based access with the clinicians who will use it, and plan the move with the clinic software switching checklist.

Bring three of your own patient pathways and watch them run on one record, from the first enquiry to the second discipline's note.

Frequently asked questions

What software does a multidisciplinary clinic need?

One system with a single patient record for every discipline, a diary that books across practitioners, referral from inside the record, access set by role and one inbox for patient messages.

Can a dietitian or physiotherapist see the doctor's notes?

That is the clinic's decision, set by role. Sharing for treatment is generally permitted, and in the United States HIPAA's minimum necessary standard does not apply to disclosures between providers for treatment.4 Sensitive note types can still be limited to a narrower circle.

How should internal referrals be measured?

As bookings. Track the share of internal referrals that become a completed appointment, by discipline, every month. In one large US study only 34.8% of referral scheduling attempts ended in a documented completed appointment, so a referral booked in the room is worth a great deal.1

Do different disciplines need different note templates?

Yes. A physiotherapy assessment and a psychology session are structured very differently, so give each discipline its own templates on the same record.

Does wai work for clinics with several practitioners and sites?

Yes. wai keeps the consult and the front desk on one record, and a patient seen at one site is known at the next.6 Admin is priced per location and starts with a demo and a scoped pilot, as published on the pricing page.

Who approves the messages patients receive?

A person at the clinic. Clinical letters go out only once a clinician has signed them, and the team approves every operational message before it sends.8 Questions about symptoms or results go to the right practitioner with the thread attached.
