Key points
- Run the switch in six phases, from planning to go-live, each with a named owner and a test of done.
- US certified record systems must let a user export all of a single patient's electronic health information, and support an export for the whole patient population.1
- HIPAA documentation must be kept for 6 years, and the AMA says immunization records should be kept indefinitely.32
- Turn reminders on from day one on the new system. Across 21 randomised studies, reminders cut no-shows from 21% to 15%.5
- Keep read access to the old system and its full export until the longest retention period that applies to your clinic has passed.
Summary
Switch in six phases. Plan what moves and who owns it, export every record in a computable format and test it, set the retention period for the old data under your country's law, run both systems in parallel for two to four weeks, train staff and tell patients, then go live on a quiet day with the old system kept read-only.
Six weeks from the export request, the clinic runs on its new system with every record intact and every future booking in place. Clinicians find each past letter where they expect it, and the old system is retired on schedule.

| Phase | Step | Owner | Done when |
|---|---|---|---|
| 1. Plan | List every data type the old system holds and mark it move, keep read-only or archive | Switch owner | Inventory signed off by a clinician and the operations lead |
| 1. Plan | Set a go-live date at least six weeks out on a low-volume day | Switch owner | Date in every clinician's diary |
| 1. Plan | Check the old contract for notice period, export terms and fees | Operations lead | Notice date and export cost known in writing |
| 2. Export | Request a full export of every patient, with the format documented | Operations lead | Supplier confirms format and delivery date in writing |
| 2. Export | Confirm the export is computable, not images of pages | Switch owner | A sample opens as structured data |
| 2. Export | Test twenty complex patients field by field in the new system | Lead clinician | All twenty match, or each gap has a fix |
| 2. Export | Store the raw export files securely, separate from the import | Operations lead | Location, access list and encryption recorded |
| 3. Retain | Confirm the retention periods that apply in your country and state | Data protection lead | Written retention policy updated |
| 3. Retain | Agree read-only access to the old system or its export for that period | Operations lead | Access route and end date documented |
| 4. Parallel | Move future appointments and check each against the old diary | Front desk lead | Every future booking matches |
| 4. Parallel | Reconcile deposits, invoices and memberships daily | Operations lead | Totals agree for seven days in a row |
| 5. Train and tell | Train each role on its own workflows with real patients | Switch owner | Each person has completed a full task unaided |
| 5. Train and tell | Tell patients twice, two weeks before and on the day | Front desk lead | Message sent with the new booking link |
| 5. Train and tell | Turn on reminders for every booking | Front desk lead | Test patient receives a reminder |
| 6. Go live | Freeze the old system and take a final export | Operations lead | Final export filed with the first |
| 6. Go live | Run a fifteen minute review each day for five days | Switch owner | Issue log empty or every issue owned |
| 6. Go live | Check the audit log exports from the new system | Data protection lead | Export opened and filed |
Decide what moves, what stays and who owns it
Inventory what the old system holds, from demographics, notes, letters, results and prescriptions to consent forms, appointments, invoices, payments, memberships, message history and the audit log. Mark each one move, keep read-only, or archive.
Name one owner for the whole switch and one for each phase. Pick a go-live date at least six weeks out, on a low-volume day, away from the end of a billing month and from any clinician's leave.
Export everything, and prove the export works
Ask the old supplier in writing for a full export, its format and the delivery date. In the US, certified record systems must let a user create an export of all of a single patient's electronic health information, without developer help, and support an export of all the electronic health information for the whole patient population.1 The files must be electronic and in a computable format, and a PDF counts only if it is machine-readable.1
Pick twenty patients with long histories, including children, a deceased patient and someone with a complex medication list, and check each one field by field in the new system. Keep the raw export files as well as the imported version as the clinic's evidence of what the old record said. For scribe notes, follow switching an AI scribe without losing records.
Retention rules outlive the old contract
Confirm your own retention periods before you cancel anything. In the US, HIPAA requires required policies and documentation to be kept for 6 years from creation or from when they were last in effect, whichever is later.3 The AMA's Code of Medical Ethics adds that physicians should keep what a new physician would reasonably need, keep immunization records indefinitely, and tell patients how to reach stored records and for how long they will be available.2
In the UK, the ICO's storage limitation principle says personal data must not be kept longer than needed, that the clinic must be able to justify how long it keeps it, and that it needs a policy with standard retention periods.4 Write or update that policy now, and record where the archived export is held and who can open it.
Run both systems in parallel
For two to four weeks, book new appointments in the new system and keep the old one open read-only for history. Move future appointments in one batch, then check every one against the old diary. Any clinician should be able to find a past letter in under a minute.
Reconcile money daily. Compare deposits, open invoices and memberships in both systems each evening until totals agree for a full week.
Train the team and tell the patients
Train each role on its own day with real patients. The front desk learns the inbox, booking and payments, and clinicians learn notes, letters and signing.
Tell patients two weeks before go-live and again on the day, with the new booking link, any new way to pay and confirmation that their record has moved with them. Switch reminders on for every booking from the first day. In a BMJ Open meta-analysis of 21 randomised studies, patients sent reminders were 23% more likely to attend and no-shows fell from 21% to 15%.5
Go live, then keep the old system read-only
On go-live day, stop new entries in the old system, take a final export and file it with the first. Hold a fifteen minute review at the end of each of the first five days and log every issue with an owner.
On wai, every message, draft, approval and signature is written to an event log the clinic can export, and signed notes and letters can be exported into other systems, so the clinic can always read its own history.76
wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. Booking reads each clinician's busy times from Google Calendar, Outlook or Apple Calendar so patients are only offered open times, and it never writes into a clinician's own calendar.
Where to start
This week, request the full export and its format from the current supplier in writing, and write down the retention periods that apply to your clinic. Then set the go-live date, name the owners and book the twenty-patient test. The implementation plan covers what comes after go-live, and wai prices are published on the pricing page.
Bring a sample export from your current system. We show you how it lands on wai, patient by patient, before you commit.
Frequently asked questions
How long does it take to switch clinic software?
Plan for about six weeks from the export request to go-live, including two to four weeks of parallel running. Most of the time goes on the export and the twenty-patient test.
How long must we keep the old records?
Can the old supplier refuse to give us our data?
In the US, certified record systems must support a full export of every patient's electronic health information.1 Elsewhere, check the contract's export terms before giving notice, and ask for the format in writing.
Should we keep the old system after go-live?
Keep read-only access or a complete, tested export for the full retention period, with a documented way to open it, so every records request can be answered.
What should we tell patients?
That their record has moved with them, the new booking link, any new way to pay, and who to contact with questions. Send it two weeks before go-live and again on the day.
