Key points
- A clinic-wide rollout runs in four stages. Owners and baselines on day 0, the front desk live in the first week, clinicians on drafted notes in week two, and a decision on day 100.
- Measure before anything changes. Eight weeks of reply times, no-shows and same-day notes give every later number its denominator.
- The front desk goes first because its gains show fastest. Across 21 randomised studies, text reminders cut non-attendance from 21% to 15%.6
- In 2025, 2% of 180 health system leaders surveyed by Deloitte had deployed AI across the whole enterprise.5
- wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. Most locations are live within a week, and the 90-day pilot ends with a measured result.1
Summary
Roll clinic AI out in four stages over 100 days. Record baselines and name owners on day 0, put the front desk live in the first week, bring clinicians onto drafted notes in week two, and decide on day 100 against the numbers written down at the start.
By day 100 the front desk answers every patient with a drafted reply the team approves, every clinician signs notes drafted during the consult, and the clinic has a measured result to decide on. The plan below gets there from a signed agreement, with an owner and a number for each stage.

| When | What happens | Owner | What to measure |
|---|---|---|---|
| Day 0 | Agreement signed, owners named, eight weeks of baselines recorded, DPIA started | Operations lead | Baseline reply time, bookings per enquiry, no-show rate, same-day notes |
| Days 1 to 3 | Hosting set up in the clinic's country, phones, WhatsApp and email brought into one thread per patient, clinician busy times read from their calendars | wai with the operations lead | Every channel arriving in one thread |
| Days 3 to 7 | Clinic voice, prices and approval map set, front desk live with drafted replies | Front desk lead | Drafts approved unchanged, median time to first reply |
| Week 2 | First two or three clinicians on drafted notes, letters and codes | Clinical lead | Notes signed the same day, edits per note |
| Day 10 | First formal review against baselines | All three owners | Every baseline line, side by side |
| Day 30 | All clinicians live, booking and payment on the clinic's own website | Operations lead | Online bookings, deposits collected, no-show rate |
| Day 100 | Decision to widen, adjust or stop | Operations lead | Revenue, hours saved, patients cared for, each against baseline |
Where clinics stand
In an AMA survey of 1,692 US physicians in January and February 2026, over 80% said they use AI in their professional work.7 Deloitte's survey of 180 health system executives in six countries, fielded in August and September 2025, found about 30% running generative AI at scale in some areas and 2% with AI deployed across the whole enterprise.5 A private clinic can get there in a quarter when each stage has a named owner and a number to watch.
Day 0, owners and baselines
Sign the agreement and name three owners. A clinical lead owns notes and anything that touches a diagnosis. A front desk lead owns messages and bookings. An operations lead owns the plan and the day 100 decision.
Pull eight weeks of baseline figures the same day from the systems the clinic already runs. Record enquiries received, median time to first reply, bookings per enquiry, the no-show rate and the share of notes signed on the day of the consult, each with its denominator.
Start the data protection impact assessment now if the clinic works under UK GDPR. The ICO says a DPIA must be carried out before processing that is likely to result in a high risk, and its examples of high risk include innovative technology and special category data such as health records.4
Week 1, the front desk goes live
wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. The same week, the clinic's email, website, WhatsApp and phone calls come into one thread per patient, with the diary alongside.2 Booking reads each clinician's busy times from Google Calendar, Outlook or Apple Calendar, so patients are only offered open times.
Next, load the clinic's voice. Prices, directions, cancellation terms and the tone the front desk already uses go in, and an approval map sets who signs off which kind of message. From then on wai drafts each reply and the team approves it before it sends.3
The pricing page states that most locations are live within a week.1 Reminders, deposits and waitlist backfill switch on with the diary, where the earliest gains show. A BMJ Open meta-analysis of 21 randomised studies found text reminders cut non-attendance from 21% to 15%.6
Week 2 and day 10, the consult
Clinicians join once the front desk is steady, starting with two or three who volunteer. Each consult produces a drafted note, letter and suggested codes, and the clinician reviews and signs every one before anything clinical is sent.3
Day 10 is the first formal review. The clinical lead reads a sample of signed notes for the edits made, the front desk lead checks how many drafts were approved unchanged, and the operations lead compares reply times with the day 0 baseline. The volunteers then train the rest.
Days 30 to 100, the whole clinic
By day 30 every clinician is drafting notes, and the booking page on the clinic's own website takes bookings and payment. Patients who book get their own Profile, so the record they see matches the one the clinic works from.
Review the event log weekly in the first month and fortnightly after, since every message, draft, approval and signature is written there and can be exported.3 Keep the same denominators as day 0.
The 90-day pilot is set up to show what changed in revenue, hours saved and patients cared for.1 Compare each line with its baseline and decide on day 100 to widen, adjust or stop. Setting the thresholds in advance is covered in our guide to pilot success criteria.
The plan on one page
The table sets out each stage, its owner and the number that shows it worked.
Where to start
Name the three owners this week, before any contract is signed, and pull eight weeks of baseline figures. Draft the approval map on paper, listing every common message type and the person who approves it.
Then request a demo with those numbers and ask to see this plan run on the clinic's own diary and messages. Before signing, work through the procurement checklist and what to move when switching clinic software.
Bring your diary and a week of messages, and see this plan mapped onto your clinic in the demo.
Frequently asked questions
How long does it take to implement AI in a private clinic?
The front desk can be live in the first week, and the wai pricing page says most locations are live within a week.1 Clinicians follow in week two, and the full measured pilot runs 90 days.
Who should own the rollout?
Three people. A clinical lead for notes and anything clinical, a front desk lead for messages and bookings, and an operations lead for the plan and the final decision.
What should we measure before going live?
Eight weeks of enquiries, median reply time, bookings per enquiry, no-show rate and the share of notes signed on the day. Record each with its denominator so day 100 compares like with like.
Do we need a data protection impact assessment?
In the UK, most clinics will. The ICO requires one before processing likely to result in a high risk, and health data processed with new technology usually meets that bar.4
Should the front desk or the consult go first?
The front desk. Its gains show within days, and clinicians then join a system the team already uses to review drafts.
Where is the clinic's data hosted?
wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. The questions to ask any vendor are in our guide to data residency.
