Key points
- At one US academic health system, 698 physicians who adopted an AI scribe produced 1.81 more relative value units a week than 867 who did not, with no difference in claim denials.4
- Uptake decides the return. In a randomised trial of 66 practitioners, 27,092 of 71,487 notes (38%) were drafted with ambient AI.2 The rollout plan has to reach clinicians who would not adopt on their own.
- Governance fits on one page, with a review policy, a consent script, templates per specialty and a monthly audit of signed notes.
- When the consult, letters, codes and front desk share one record, the work a note creates is already moving.
- wai runs the whole clinic on that record, with the clinician signing every clinical document and the team approving every patient message.6
Summary
Roll out an AI scribe across the clinic with shared rules. Agree a review policy, a consent script and a template per specialty, then measure uptake and time per note from the first month. Choose a platform where the note, letters, coding and front desk share one patient record.
A clinic-wide rollout gives every clinician the evening back and moves each consult straight into letters, codes and the next booking. The evidence below shows what that returns at clinic scale, and the rollout that captures it.

| Decision | What to agree | Owner | Measure |
|---|---|---|---|
| First group | Three to five clinicians who want to start, across at least two specialties | Clinical lead | Share of their visits drafted with the scribe |
| Review policy | Every note read and signed by the clinician who saw the patient | Clinical lead | Monthly audit of a sample of signed notes |
| Consent | One script and one written notice for every clinician | Operations lead | Consent line present in audited notes |
| Templates | Note and letter templates for each specialty, built before each group starts | Each specialty lead | Edits needed per note in the first month |
| Front desk link | How follow-ups, letters and codes move from the note to the team | Operations lead | Time from signed note to booked follow-up |
| Calendars and systems | Which calendars and record systems the platform connects to | Operations | Double entry removed |
| Data and hosting | Where records are hosted, who can see them, audit log and export | Data protection lead | Written answers on file before go-live |
| Economics | Baseline time in notes, same-day closure and visits per week | Finance lead | Change after 30 and 90 days |
The return at clinic scale
In a 2026 study of 1,565 physicians at one US academic health system, the 698 who adopted an ambient scribe produced 1.81 more relative value units a week than the 867 who did not, saw 0.80 more patients a week, and showed no difference in the share of claims denied.4 The authors put the gain at $3,044 a year per physician at 2025 Medicare rates, and noted that early adopters may differ from others.4
In a pre-post study of 46 clinicians across 17 specialties, time in notes per appointment fell 20.4%, from 10.3 to 8.2 minutes, and same-day note closure rose from 66.2% to 72.4%.3 Across 263 clinicians in 6 US health systems, time spent documenting after hours fell by 0.90 hours on average after 30 days.1
Multiply the per-clinician gain by the clinicians who actually use the tool, which is often fewer than the seats bought. Our business case guide sets out the arithmetic.
Uptake decides the return
Across 66 practitioners in ambulatory clinics in two US states, 38% of 71,487 notes were drafted with ambient AI, and time spent on notes still fell by 0.36 hours a day while diagnostic billing codes improved.2 At the health system above, 44.6% of physicians chose to adopt.4
Start with the clinicians keenest to use it, share their results internally after a month, then bring in the next group with templates already built for their specialty.
Governance on one page
Agree four rules before go-live and keep them to one page. The clinician who saw the patient reviews and signs the note. Patients hear one consent script, each specialty has its own note and letter templates, and the clinical lead audits a sample of signed notes each month.
The consent wording is in our consent form template, and the review routine is in who signs an AI clinical note.
One platform for the whole visit
On one platform the note sits with everything around it. The booking, deposit and intake form before the consult, and the referral letter, code, invoice and follow-up booking after it, share the same patient record.
Deloitte's 2026 outlook, from 180 health system executives in six countries, found about 30% operating generative AI at scale in select areas of their organisations, and the rest earlier on.5 One platform across the consult and the front desk takes a clinic from those pockets to the whole day.
How wai runs a whole clinic
wai Clinicians drafts the note during the consult and the letter after it, and the clinician reviews and signs.6 wai Admin handles patient messages across email, web, WhatsApp and calls in one thread per patient, with booking, reminders, deposits, invoices, codes from consult notes and claims prepared for insurers, and staff approve before anything is sent.76
Every site and every clinician works on one patient record, so a patient seen at one site is known at the next.6 wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. Admin is priced per location and starts with a demo and a scoped 90-day pilot, with plans published on the pricing page.8
Where to start
Record a baseline for every clinician this month. Measure time in notes per appointment, same-day note closure and after-hours documentation, using whatever reporting your current system offers.
Then pick three to five clinicians, agree the one-page governance, build their templates and run a scoped pilot against targets set in advance. Our procurement checklist covers the contract and data questions to settle before the rest of the clinic joins.
Bring two clinicians from different specialties and see both templates and the shared record live.
Frequently asked questions
Is it worth buying an AI scribe for every clinician at once?
Start with a first group, then expand. Uptake varies even in trials, with 38% of notes drafted by ambient AI in one randomised study, so a staged rollout puts seats where they are used.2
What return can a clinic expect?
One US study found 1.81 more relative value units a week per adopting physician, about $3,044 a year at 2025 Medicare rates, with no difference in denials.4 Results depend on uptake and specialty, so measure your own baseline.
Do different specialties need different templates?
Yes. Build note and letter templates for each specialty before that group starts.
Who is responsible for notes drafted across the clinic?
Each clinician for their own notes. The clinic is responsible for the policy, the consent process and the monthly audit.
What does one platform add to the scribe?
The letters, codes, follow-ups and patient messages a note creates move on the same record without being re-entered. With wai, clinicians sign clinical documents and staff approve patient messages before anything is sent.6
How is wai priced for a clinic?
Clinician plans and Admin plans are published on the pricing page. Admin is priced per location and starts with a demo and a scoped 90-day pilot.8
