Key points

Summary

A London clinic can use AI for notes and patient messages once its CQC registration covers the care it gives, a DPIA names the lawful basis for health data, and patients have agreed to recording in line with GMC guidance. A person at the clinic approves every AI draft before it goes out, and the clinician signs each note.

Every self-pay enquiry answered overnight in the patient's language, and the consultant's lists full by morning. Self-pay admissions are at their highest level, and England's rules for using AI to meet that demand are already settled.1

The private care market in London

The Private Healthcare Information Network recorded 247,495 private admissions across the UK in the first quarter of 2026, just above the previous record quarter.1 Insurance funded 69% and self-pay 31%, and self-pay admissions rose 7.7% on a year earlier to their highest level.1

London shows the shift clearly. Insured admissions fell in London over the year, while self-pay admissions rose in every English region.1 The UK had 11,600 consultants active in private practice in the quarter, more than in any quarter of the past five years.1 A fast reply with the deposit taken at booking turns more of those enquiries into booked appointments.

The rules a London clinic works under

Registration. Any person or organisation providing a regulated activity in England must be registered with the Care Quality Commission, or commits an offence, and registration is granted for named activities at named locations.2 Adding AI changes how the clinic works, so check the statement of purpose still describes it. The guide to AI for private clinics covers the wider picture.

Data protection. UK GDPR treats data concerning health as special category data that needs extra protection, and the ICO notes its guidance is under review after the Data (Use and Access) Act.3 The ICO lists innovative technology, including AI, as requiring a data protection impact assessment when combined with other high-risk criteria.4 Complete the DPIA before the first patient record moves.

Recording. The GMC's guidance on visual and audio recordings of patients sets eight principles of good practice and explains when a doctor needs specific consent, with a separate section on recording telephone calls.5 Tell every patient before the AI scribe runs and note the answer in the record.

Medical devices. The MHRA publishes guidance on when software and AI qualify as a medical device, last updated in February 2025.6 Check the product's intended purpose statement against that guidance.

Hosting. wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. wai is compliant with these rules and built for clinics in London. A clinic in England has its records hosted in the United Kingdom.

The languages London patients write in

78.4% of London residents aged three and over had English as their main language in Census 2021, about 6.7 million people, the lowest share of any English region.7 London also had the highest share of people with Romanian as their main language, 1.9% or 159,000, rising to 7.5% in Harrow.7

International patients add further languages to the inbox. wai works in any language and is built for Arabic-first care.8 Every reply goes out in the patient's language after a member of the team has approved it.

What a demo shows in London

The demo opens on a WhatsApp message in Arabic sent at 11pm by a family staying in Knightsbridge, asking for a consultant appointment before they fly home on Friday. wai reads it, checks the consultants' busy times in Google Calendar or Outlook so only open slots are offered, and drafts the reply in Arabic. A member of the team approves it at 7.30am, and the family pays a deposit in pounds through the clinic's own Stripe account.

A missed call to the clinic during a busy list becomes a drafted text the team approves in the same thread. After the consult, the consultant reviews and signs the drafted note and the referral letter.

Bring the clinic's last week of enquiries, in the languages they arrived in.

For hospital groups and insurers

London hospital groups and insurers can have wai built around a single pathway, such as self-pay outpatient booking, under their own name, with data and model rights in the contract. The organisations page sets out the steps.

Where to start

First, check the CQC registration and statement of purpose cover the services the AI will support.2 Second, complete the DPIA, naming the lawful basis and the condition for processing health data, and list every subprocessor.34

Third, add a recording sentence to the consult script and intake form in line with GMC guidance, using the consent form template.5 Fourth, count last month's enquiries by language and by hour, and bring the count to the demo. This page is general information as read on 14 October 2026, and the clinic's own adviser should confirm how it applies.

Bring last month's enquiries, counted by language and hour, and see the lists they would fill.

Frequently asked questions

Does a London clinic need CQC approval to use AI?

The CQC registers providers for regulated activities and locations, rather than software.2 The clinic must be registered for the care it gives, and its statement of purpose should reflect how it works.

Does a private clinic need a DPIA for an AI scribe?

The ICO lists innovative technology, including AI, as requiring a DPIA when combined with other high-risk criteria, and health data is special category data.43 Most clinics should complete one before go-live.

Where are a UK clinic's records hosted?

wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. A clinic in England has its records hosted in the United Kingdom.

Do patients have to consent to an AI scribe recording?

The GMC's guidance on audio recordings explains when specific consent is needed.5 Telling every patient and noting the answer is the safe standard, covered in the guide to AI scribe consent.

Is an AI scribe a medical device in the UK?

It depends on its intended purpose, and the MHRA's software and AI guidance sets out how that is judged.6 Start from the product's intended purpose statement.

Can wai reply to patients in Arabic?

Yes. wai works in any language and is built for Arabic-first care.8 The team approves each reply before it is sent.
