Summary
A Houston clinic can use AI for notes and patient messages in 2026 when it keeps records physically in the United States, tells patients when AI is part of their care and has a clinician review every record AI creates. Those duties come from Texas SB 1188 and the Texas Responsible AI Governance Act, alongside HIPAA and the Texas Medical Records Privacy Act.
Houston clinics now answer patients in Spanish, Vietnamese and every language the county speaks, at any hour, with the clinician still signing every note. They work beside the Texas Medical Center and under the AI and record storage rules Texas brought in on 1 January 2026.

The private care market in Houston
The Texas Medical Center describes itself as the largest medical complex in the world, with 10 million patient encounters a year, 750,000 emergency visits, 10,000 patient beds and more than 120,000 employees.2 It also houses the world's largest children's hospital and the world's largest cancer hospital.2
Around that core sit the independent practices, specialist groups and aesthetic clinics of a county of 4.5 million residents aged five and over.1 Patients referred into the medical center return to these clinics for follow-up, and a fast first reply brings them back.
The rules a clinic in Houston works under
HIPAA is the federal floor. Physicians are licensed by the Texas Medical Board, whose rules sit in Title 22, Part 9 of the Texas Administrative Code.7
SB 1188 created Chapter 183 of the Health and Safety Code. Its residency rule, section 183.002, covers any record of a Texas patient stored on or after 1 January 2026, cloud copies included.3 Its AI rule, section 183.005, ties every AI-created record to the Texas Medical Board's review standards and to a disclosure to the patient.3 Two more sections shape a Houston family practice. Section 183.004 bars storing a patient's credit score or voter registration status in the record, and section 183.006 requires record systems to give a parent or guardian complete access to a minor's record.3
HB 149, the Texas Responsible Artificial Intelligence Governance Act, reaches any AI system used in relation to a health care service or treatment. The patient must be told by the date the service is first provided, even where the use of AI would be obvious to a reasonable person.4
Chapter 181, the Texas Medical Records Privacy Act, covers any person who assembles, stores or transmits protected health information, clinics and website operators included.5 The attorney general can seek up to $5,000 per negligent violation, $25,000 per knowing violation and $250,000 where information was used for financial gain, up to $1.5 million a year for a pattern, and the licensing agency can add discipline.5
Texas lets a party to a conversation record it under Penal Code section 16.02.6 For telehealth patients calling from other states, see the guide to US recording laws.
wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. wai is compliant with these rules and built for clinics in Houston. Houston records stay in the United States, as section 183.002 requires.
The languages Houston patients write in
Of 4,507,628 Harris County residents aged five and over, 55.0% speak only English at home and 35.3% speak Spanish.1 The next largest are Vietnamese at 90,861 people, Chinese including Mandarin and Cantonese at 49,049, Arabic at 28,366 and Tagalog at 20,653.1
Fewer than half of Vietnamese speakers in the county, 35,528 of 90,861, report speaking English very well.1 wai works in any language, so a message is read in the language it arrives in and answered with a draft the team approves.8
What a demo shows in Houston
The demo starts with a Vietnamese WhatsApp message sent at 9pm from Alief, asking whether a grandmother can see the doctor before her son's shift on Thursday. wai matches it to the family's record, reads the clinicians' busy times from Google Calendar or Outlook and drafts a reply offering an open slot. The front desk approves it the next morning.
Next comes a Spanish voice note about a follow-up after a Medical Center referral, and a missed call from the Katy rooms that becomes a drafted text within minutes. In the consult, wai drafts the note while the clinician talks, and the clinician signs it before the patient leaves, the review SB 1188 asks for.3
For health systems and insurers in Houston
Hospital systems and county health services need AI disclosures and a full audit trail across many sites. wai builds documentation and patient contact around one defined pathway, with data, configuration and model rights agreed in the contract, as set out on the page for health systems and governments.
Where to start
First, write the HB 149 disclosure in English and in the languages your patients write in, Spanish and Vietnamese first, and give it to every patient before their first AI-assisted visit.4 Second, confirm in writing where every record and backup is stored, to meet section 183.002.3
Third, check that the record system gives parents full access to a minor's record and holds no credit scores, per sections 183.004 and 183.006.3 Fourth, count last month's messages by language as the pilot's baseline, then book a Houston demo and bring it. This page is general information on Texas law as read on 14 October 2026, and the clinic's own counsel should confirm how it applies.
Bring a week of your Houston messages, in Spanish, Vietnamese or any language they arrive in, and see the drafts your team would approve.
