HIQA inspection checklist
1Governance
- A named person with overall responsibility, ready to receive the announcement.
- A current, public statement of purpose under Standard 5.3.
- Governance minutes that show quality and safety data read and acted on.
- Last inspection's compliance plan, with every action dated and closed.
2Self-assessment
- Each core standard self-assessed against the lines of enquiry in the judgment framework.
- Evidence filed per standard, so the five-day document request is a retrieval.
- Leads named for infection prevention, medication safety, the deteriorating patient and transitions of care.
- No identifiable patient information in anything sent to HIQA.
3Patient records
- Ten records a stranger could follow, signed and timed.
- Consent recorded, with the discussion in the note.
- Handover and discharge letters complete and sent.
- An access log for every record.
4Incidents and complaints
- Notifiable incidents sent to HIQA within seven days through NIMS.
- Open disclosure meetings held and the written statements kept.
- Complaints answered promptly, with the outcome recorded.
- Learning from incidents shared, with what changed.
