Key points
- AI notes and front desk drafts arrive without a migration. The existing system stays the legal record, and signed documents flow into it.
- Decide the record of truth first, then how each signed document reaches it, by export or by write-back where the system supports it.
- Data flow between systems is the work to plan. Only 8% of 8,122 US family physicians reported an ideal experience receiving outside test results in their EHR.1
- Signed notes and letters can be exported from wai into any system, and the integrations page lists every record system wai is built to connect to.4
Summary
Yes. Keep the record system as the legal record and add AI that drafts notes, letters and front desk replies around it, with each signed document exported or written into that record. The work is deciding which system holds the record of truth and how each document reaches it.
Notes and letters finished in the consultation, and every signed document filed in the record the team already knows. The record system stays exactly where it is, with no migration and no new chart to learn.

| Flow | What moves | Who does it | Works with |
|---|---|---|---|
| Export and file | Signed notes and letters, out of the AI tool | Clinician signs, a named person files the same day | Any record system |
| Write-back | Signed notes and letters, straight into the patient's record | Clinician signs, the connection files | Systems the vendor lists as supported |
| History read before the visit | Patient history, into the clinician's briefing | Read-only, the record stays as filed | Systems with a supported read, often over FHIR |
| Calendar availability | Each clinician's busy times, into booking | Read only, set once | Google Calendar, Outlook or Apple Calendar |
| Front desk threads | Messages, bookings and payments, kept beside the record | Team approves every reply | Runs alongside any record system |
Keep the record, add the drafting
The data work sits between systems. In a survey of 8,122 US family physicians fielded from November 2023 to October 2024, ideal interoperability experiences ranged from 19% for encounter documents down to 8% for test results from outside hospitals.1
In 2023, 43% of US non-federal acute care hospitals routinely engaged in interoperable exchange, 71% reported routine access to clinical information from outside providers, and 42% said their clinicians routinely used it in care.2 Keeping the record and adding the drafting in front of it brings the AI gains forward.
Decide the record of truth first
Write down which system holds the legal record for each type of document. For most clinics adding AI, the existing record system stays the record of truth for notes, letters and results, and the AI tool holds drafts and signed copies.
Then set one rule for every document type, signed in the AI tool and filed in the record the same day. Clinicians always find the latest signed note in the record, and the record holds signed documents only.
How signed notes and letters reach the record
Export works with any system. The clinician signs the note or letter, it is exported, and it is filed against the patient in the existing record, a short end-of-session routine for whoever files correspondence today.
Write-back files signed documents straight into the patient's record where the system supports it. Ask each AI vendor for its list of supported systems and what moves in each direction, as covered in our guide to AI scribe and EHR integration.
A history read runs the other way. The AI tool pulls the patient's history from the record before the visit, so the clinician starts the consultation already briefed.
The standard behind history reads
Most modern history reads use HL7 FHIR, a standard for exchanging healthcare information electronically, where all exchangeable content, from a patient to an observation, is defined as a resource.3
Ask whether a read is live for your own system, which resources it covers and whether it is read-only, so that the record changes only when a person files a signed document.
The front desk can run alongside
Messages, bookings, reminders and deposits can run in a front desk system beside the record, with each clinician's busy times read from their Google Calendar, Outlook or Apple Calendar, and every reply drafted for the team to approve.
Anything clinical that arrives at the front desk passes to a clinician with the thread attached, and the signed outcome is filed in the record like any other document.
What wai connects to
wai is built to connect to two large US hospital records systems, both listed on the integrations page. We set up your clinic's connection with you. That connection is a read-only pull of a patient's history over FHIR R4, so the clinician is prepared before the visit.4 The integrations page also lists the UK practice system wai is built to write signed notes and letters into.4
For every other system, signed notes and letters can be exported from wai into it.4 wai drafts the note during the consultation and the letter after it, and the clinician reviews and signs before anything is filed.5 Each clinician's busy times are read from their own calendar, so patients are offered only open times.4
Where to start
Name the record of truth for each document type and the person who files signed documents today. Send each AI vendor your record system's name and ask for its supported flows in writing.
Run two weeks with export and same-day filing, and measure how long filing takes per session. Clinicians finish their notes and letters in the consultation, and the record the team trusts stays complete.
Tell us which record system you run. We show you the exact flow, live, in fifteen minutes.
Frequently asked questions
Do we have to replace our EHR or practice system to use AI?
The existing system stays the legal record, with AI-drafted notes and letters signed and then exported or written into it.
How do we avoid typing everything twice?
Set the rule that every document is signed in the AI tool and filed in the record the same day. Export or write-back moves the signed document as it is.
Which record systems does wai connect to?
wai is built to connect to two large US hospital records systems, both listed on the integrations page. We set up your clinic's connection with you. The integrations page lists every system and what moves in each direction, and signed notes and letters can be exported into any system.5
Does the AI change anything in our record on its own?
History reads are read-only, and notes and letters reach the record only after a clinician has signed them.
What if our system is missing from a vendor's list?
Export works with any system. Name the system in the demo, and ask whether a write-back or a history read is possible for it.
Can the front desk use AI without touching the record system?
Yes. Messages, bookings and payments can run alongside the record, with the team approving each drafted reply and anything clinical passed to a clinician.
