Key points

Summary

A Washington DC clinic can use AI for notes and the front desk in 2026 under HIPAA, the District's one-party recording rule and its breach law, with every physician licensed by the DC Board of Medicine. Keep a clinician signing every note, a person approving every patient message and records hosted in the United States.

DC clinics answer every enquiry by the next morning, in the patient's own language, and finish notes before the last patient leaves. The District sets clear rules for that work, from licensing at DC Health to a 50-resident threshold for breach notice.

The private care market in Washington DC

The District had 681,294 residents in the 2020 to 2024 American Community Survey, and only 3.6% of its civilian noninstitutionalized population was uninsured.7 12.8% of residents are 65 or older and 14.3% were born abroad, so most patients arrive insured and many write in a second language.7

Many physicians practise on both sides of the District line. The DC Board of Medicine offers a DMV reciprocity pathway for US and Canadian educated physicians already licensed in Maryland or Virginia, and it has issued licences through the Interstate Medical Licensure Compact since 12 June 2023.1

The rules a DC clinic works under

HIPAA sets the federal floor. The District adds its own licensing, breach and recording rules on top.

Licensing sits with the DC Board of Medicine inside DC Health. DC Health is moving every licence to a birth-month renewal cycle, and licensees still owe two credits of LGBTQ cultural competency and 10% of their continuing education on public health priorities.1

The District's data breach law, DC Code section 28-3851, lists medical information, health insurance information, genetic information and biometric data as personal information.3 Section 28-3852 requires notice to each affected resident in the most expedient time possible, and written notice to the Office of the Attorney General when a breach affects 50 or more District residents, no later than the residents are told.4

Recording is governed by DC Code section 23-542, which permits interception where the person is a party to the communication or one party has given prior consent.2 Patients from Maryland and Virginia call DC clinics every day, and the guide to US recording laws covers calls that cross a state line.

The Department of Health Care Finance names CRISP DC as the competitively selected District Designated HIE since April 2020, supporting exchange among District Registered HIE entities and participating organisations.5 wai is built to connect to CRISP DC. We set up your clinic's connection with you.

wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. wai is compliant with these rules and built for clinics in Washington DC. For a DC clinic that means the United States, and for Checkout a business associate agreement is signed before anything goes live.8

The languages DC patients write in

122,363 District residents aged five and over speak a language other than English at home, 19.1% of that population.6 Spanish leads at 60,423 speakers, followed by French at 10,192 and Amharic, Somali or other Afro-Asiatic languages at 8,379, with Chinese at 5,861.6 5.6% of residents speak English less than very well.6

wai works in any language.9 Intake questions can be answered by voice in any language, and the clinician reads them in English.10

What a demo shows in Washington DC

The demo opens on a WhatsApp message sent at 10:15pm in Spanish from a mother in Columbia Heights, asking for a Saturday appointment for her daughter. wai drafts the reply in Spanish in the patient's thread, offering only times the clinicians' Google Calendar, Outlook or Apple Calendar show as available, and the coordinator approves it at 8am.

The booking link opens on the earliest time and completes in three taps, with a deposit in US dollars by card or Apple Pay through the clinic's own Stripe account.8 The patient gets a confirmation, a receipt and an intake questionnaire. A patient answers the intake by voice in their own language and the clinician reads it in English before the visit.10

After the consult the clinician reviews the drafted note and referral letter, edits and signs it. Bring one real Spanish enquiry from last month to the call.

For health systems, agencies and insurers in Washington DC

Hospital groups, federal and District agencies and health plans in Washington commission a patient pathway of their own on wai, carrying their own name. Ownership of data, configuration and models is written into the contract before the first build, which is described on the organisations page.

Where to start

First, add the 50-resident Attorney General notice to the clinic's breach plan, with the AG's contact details beside it.4 Second, write a one-line recording notice for the consult room and the phone line, using the consent form template.

Third, confirm every physician's licence is current under the birth-month cycle at DC Health.1 Fourth, tally last month's enquiries by language and time of day and bring that tally to the demo. The clinic's own counsel confirms how it applies.

Send us last month's enquiries by language and hour, and we show the Saturday they would have filled.
