Key points

Summary

A practice in Umhlanga or Ballito can use AI to draft notes and patient replies under POPIA and HPCSA Booklet 20, as long as a registered practitioner makes and signs every clinical decision. Private health care facilities are licensed by the KwaZulu-Natal Department of Health, and with wai records stay in South Africa.

Every message from La Lucia to Salt Rock answered the same day, in isiZulu or English, and every note signed before the doctor heads home along the M4. In eThekwini, 16.6% of residents belonged to a medical aid scheme in 2024.2

The private care market in Umhlanga and Ballito

Umhlanga lies in the eThekwini metro and Ballito in the KwaDukuza municipality up the north coast. The General Household Survey 2024 found 16.6% of eThekwini residents on a medical aid scheme, against 10.2% across KwaZulu-Natal and 15.5% nationally.2 An estimated 564,000 KwaZulu-Natal households said they would go first to a private doctor or specialist, and 695,000 to some part of the private sector.2

Census 2022 counted 12,423,907 people in KwaZulu-Natal, the second largest province after Gauteng.1 The province is home to 1,157,542 Indian or Asian South Africans, about two thirds of the national total of 1,697,505.1

The rules a practice on the north coast works under

The KwaZulu-Natal Department of Health licenses private health care facilities against Regulation R158 of 1980 and the Norms and Standards Regulations, and licenses only those that show a high level of compliance.4 A new facility or an extension needs an application fee of R10 000, a site-specific newspaper advert with 30 days for objections, community endorsement and a municipal approval letter.4

Every doctor on the north coast is registered with the Health Professions Council of South Africa, whose Booklet 20 holds practitioners ultimately responsible for their use of AI tools and says those tools are there to assist and complement clinical care.5 It also asks that AI systems be tested for usability by people who reflect the practice patterns of the end user.5

POPIA section 71 says a patient may not be subject to a decision with legal consequences or substantial effect based solely on automated processing that profiles them, health included.3 Where an exception applies, the patient must be able to make representations and receive enough information about the logic behind the processing.3 The Information Regulator, an independent body set up under section 39 of the Act, enforces it.6

wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. wai is compliant with these rules and built for clinics in Umhlanga and Ballito. Every message, draft, approval and signature is written to an event log the practice can export.7

The languages north coast patients write in

Census 2022 found isiZulu the language most often spoken at home by 80.0% of KwaZulu-Natal residents, with English second at 14.4% and isiXhosa at 3.1%.1 Nationally isiZulu also leads, at 24.4%.1

wai works in any language. A patient in Tongaat can answer intake questions by voice in isiZulu, and the doctor reads the answers in English before the visit.8 Reception approves every isiZulu or English draft before it goes out.

What a demo shows in Umhlanga and Ballito

A WhatsApp message in isiZulu arrives at 20:15 from a grandmother in Ballito asking whether her blood pressure review can move to Friday morning. wai keeps it in her thread, reads the GP's busy times from Google Calendar, Outlook or Apple Calendar, and drafts an isiZulu reply with the first two open times. Reception approves it at 7:45.

In the Umhlanga Ridge rooms, a visitor from Johannesburg books a dermatology consult on the practice website in three taps, pays a deposit in rand by card through the practice's own Stripe account, and receives a confirmation, a receipt and an intake questionnaire.9 In the consult wai drafts the note, the dermatologist edits and signs it, and the medical aid claim is prepared from the signed note for the team to check.

For health systems, government and insurers on the north coast

Hospital groups, medical schemes and the provincial department serving the north coast can commission a system built on wai for one pathway, such as chronic medication reviews for patients outside the metro, with records hosted in South Africa and data, configuration and model rights written into the contract. See wai for organisations.

Where to start

First, if the practice is building or extending a facility, open the provincial licensing file early, since the 30-day advert and the municipal letters come before approval.4 Second, write down where AI assists the doctor and who decides, so the responsibility Booklet 20 places on the practitioner is clear.5

Third, check that no patient decision rests solely on automated processing, in line with POPIA section 71.3 Fourth, count last month's enquiries by language and town, from Umhlanga to Ballito, then book a north coast demo and bring that count. Who signs an AI clinical note covers the sign-off.

Bring a week of north coast enquiries, in isiZulu and English, and see the drafts your reception would approve.
