Summary
A Lugano practice can use AI for notes and patient messages in 2026 when the patient agrees to the recording, the doctor signs each note and the record keeps every consultation date for at least ten years. The Ticino health law and the cantonal health department set the practice rules, the federal FADP governs the data and e-Health Ticino runs the electronic patient record for the canton.
Lugano practices on wai answer the patient who works in the city and lives across the border before that patient sets off in the morning, and the doctor signs the day's notes before closing. Ticino law and the federal FADP set the terms, and the canton's statistics office shows who is writing in.

The private care market in Lugano
Ticino had 357,720 permanent residents in 2023, 4.0% of the Swiss population, and 103,926 of them were foreign nationals, 29.1% of the total.1 The Luganese region held 149,457 of those residents, 99,930 of them Swiss citizens.1
Italian nationals were by far the largest foreign group in the canton at 63,106, followed by Portuguese at 6,752, German at 2,979 and Ukrainian at 2,607.1
The working population extends across the border. In 2023 Ticino counted 110,600 employed Swiss residents, 49,700 employed foreign residents and 78,800 cross-border workers, so just over five in ten people working in Ticino were foreign nationals, against about three in ten nationally.1 Every one of those cross-border workers is a potential patient who books from outside Switzerland.
The rules a Lugano practice works under
Every health activity in Ticino needs an authorisation. Article 53 of the Legge sanitaria puts all health activity in the canton under authorisation and supervision, article 55 makes the Department the competent authority for authorisations to practise, and the Department of Health and Social Affairs holds that role.2 The cantonal doctor supervises the practice of the health professions under article 26.2
Article 67 sets the record. Every health professional working in the ambulatory sector keeps a record for each patient showing at least the patient's details, the treatment, the services provided and the dates of the start and end of treatment and of every consultation, with the diagnosis and the medicines prescribed.2 The record is kept for at least ten years from the end of treatment, and patients may, on written request, consult it, take a copy and ask for errors to be corrected.2
The federal FADP governs the data. Article 19 asks the practice to inform patients appropriately whenever it collects personal data, and article 25 lets any person ask whether their data is being processed and receive the information needed to exercise their rights.3 The Federal Data Protection and Information Commissioner, the FDPIC, is the supervisory authority.4
Article 179ter of the Criminal Code punishes anyone who records a non-public conversation they take part in without the other participants' consent, on complaint, with up to one year or a monetary penalty.5 The doctor asks before the scribe starts, and the consent form template gives the wording.
e-Health Ticino, an association of the main actors in the cantonal health system founded in 2016, supports affiliated providers with the CIP, the electronic patient record.9 Anyone in Switzerland can open a CIP regardless of their health, and professionals file documents such as discharge reports, laboratory results and prescriptions in it.6 The Confederation plans to replace the CIP with a new electronic health record, and the current CIP stays fully functional until the new law takes effect.7 wai is built to connect to the CIP. We set up your clinic's connection with you.
wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. wai is compliant with these rules and built for clinics in Lugano. Ticino patient records stay inside Swiss borders.
The languages Lugano patients write in
In the 2022 structural survey, 88.1% of Ticino residents named Italian as a main language, 13.9% another national language and 16.8% one or more foreign languages, with up to three answers allowed.1 At work, 92.4% of employed residents used Italian, and 58.4% used Italian alone.1
wai works in any language, so the secretariat sees each draft in the patient's own language and approves it before it is sent.8
What a demo shows in Lugano
The demo opens on a WhatsApp sent at 20.40 by a cross-border worker who has an office job in Lugano and wants a cardiology check before her shift on Thursday. A second message is an email from a retired couple in Castagnola asking to move their joint follow-up to the following week.
wai places each message in the patient's thread, reads the cardiologist's busy times from Google Calendar, Outlook or Apple Calendar so only open slots are offered, and drafts both replies in the language they arrived in. The secretary approves them at 7.30. Her deposit in Swiss francs is paid by card or Apple Pay into the practice's own Stripe account, and a confirmation, a receipt and the intake questionnaire reach her phone.
After the visit the cardiologist edits and signs the note, with the consultation date already in place as article 67 asks, and the letter is ready for the patient's CIP.2 Bring a week of real messages and judge the drafts against your own.
For health systems, governments and insurers in Ticino
Ticino's hospital networks, insurers and cantonal services can run their own branded system on wai, with every record kept in Switzerland. The opening project is one pathway measured against goals set at the start, and the contract states who owns the data, the configuration and the models, as the page for health systems and governments describes.
Where to start
First, confirm that every clinician who will sign notes holds an authorisation to practise from the Department.2 Second, add the AI notice to the privacy statement and the booking page, as article 19 of the FADP expects.3
Third, check that each signed note carries its consultation date and that retention runs at least ten years from the end of treatment.2 Fourth, write the consent line each doctor says before recording.5 Split last month's messages by hour and by whether the patient lives in Switzerland or across the border, and bring that split to a Lugano demo. The Ticino and federal texts cited here were read on 15 October 2026, and a local adviser should confirm how they apply to the practice.
Send us last month's messages by hour, and we show how many your secretariat could have booked before the morning traffic at the border.
