Key points

Summary

A practice in Stellenbosch, Paarl, Franschhoek or Worcester can use AI to draft notes, follow-ups and patient replies in 2026 under POPIA and the HPCSA's ethical guidelines. The doctor signs every note, telehealth consults carry express consent, recall messages respect section 69, and records stay in South Africa.

Winelands practices now answer a patient on a farm outside Ceres the same day, follow up after a video consult without a second drive into town, and send doctors home with every note signed. wai drafts the replies and the notes, a person at the practice approves or signs each one, and the records stay in South Africa.

The private care market in the Cape Winelands

Census 2022 counted 862,703 people across the Cape Winelands district's five municipalities.1 Drakenstein, which includes Paarl, had 276,800, Breede Valley around Worcester 212,682, Stellenbosch 175,411, Witzenberg around Ceres 103,765 and Langeberg around Robertson 94,045.1 Breede Valley grew fastest, at 2.4% a year from 2011, while Witzenberg and Langeberg shrank.1

Private cover runs high in the province. The General Household Survey 2024 found 25.4% of Western Cape residents on a medical aid scheme, about 1.9 million people and the highest share of any province.3 The Western Cape Department of Health and Wellness runs the district's public hospitals in Paarl, Stellenbosch, Worcester, Ceres and Robertson.7

The rules a practice in the Cape Winelands works under

The Health Professions Council of South Africa registers every doctor here, and its Booklet 10 on telehealth, revised in December 2021, says no practitioner may practise exclusively through telehealth, and that it should preferably build on an established relationship with the patient.6 The practitioner in charge obtains express informed consent, written or recorded orally, naming the practitioner and practice number, the types of transmission consented to, and how recordings are stored and encrypted.6 A copy of the consent form stays in the patient's record.6

The Protection of Personal Information Act 4 of 2013 limits how long records are kept. Section 14 allows retention beyond the purpose only where law, a contract, a lawful need or the patient's consent requires it, and records that are no longer authorised must be destroyed or de-identified so they cannot be reconstructed.4 Section 69 bars direct marketing by SMS, email or other electronic message without consent, except to existing patients for the practice's own similar services, with a way to object that carries no fee in every message.4

The Information Regulator is the independent body established under section 39 of POPIA, accountable to the National Assembly, and monitors and enforces compliance by public and private bodies.5

wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. wai is compliant with these rules and built for clinics in the Cape Winelands. The practice can export the full history of every thread, draft and signature.

The languages Winelands patients write in

Census 2022 found Afrikaans the home language of 10.6% of South Africans, of 41.2% in the Western Cape and of 54.6% in the neighbouring Northern Cape, while isiZulu led nationally at 24.4% and isiXhosa followed at 16.3%.2 In the Cape Winelands district, 54.4% of residents described themselves as coloured and 29.1% as black African.1

wai works in any language. A farm worker can answer intake questions by voice in Afrikaans or isiXhosa, and the doctor reads the answers in English before the visit.8 Replies are drafted in the patient's own language, and nothing goes out until someone at the practice approves it.

What a demo shows in the Cape Winelands

A WhatsApp message in Afrikaans arrives from a patient on a fruit farm outside Worcester asking whether her repeat prescription review can be done without the drive into town. wai keeps it in her thread, reads the GP's busy times from Google Calendar, Outlook or Apple Calendar, and drafts a reply offering a video slot and the telehealth consent wording. Reception approves it before lunch.

On the video call the scribe drafts the note, the GP edits and signs it, and the consent she gave in her thread stays on file, as Booklet 10 asks.6 The reminder before the next visit is drafted for reception to approve, and the patient gets a wai Profile for her letters and results, with permissions she can revoke.

For health systems, government and insurers in the Cape Winelands

The provincial health department, medical schemes and hospital groups serving the Winelands can commission a system built on wai for one defined pathway, such as follow-up for patients far from town, with records hosted in South Africa and data, configuration and model rights written into the contract. See wai for organisations.

Where to start

First, write the telehealth consent wording to Booklet 10, in Afrikaans, isiXhosa and English, using the consent form template as a base.6 Second, split recall and reminder messages from promotional ones, and give every promotional message an opt-out that carries no fee under section 69.4

Third, set a retention and destruction schedule under section 14.4 Fourth, count last month's enquiries by town, language and hour, then book a Cape Winelands demo and bring that count.

Bring a week of Winelands enquiries, in Afrikaans, isiXhosa and English, and see the drafts your reception would approve.
