Summary
A Cape Town practice can use AI to draft notes and patient replies today, as long as patients are told AI is used, the doctor makes and signs every decision and records stay in South Africa. The rules that matter are POPIA, enforced by the Information Regulator, and the HPCSA's Booklet 20 on the ethical use of AI.
Cape Town practices now answer every message the same day, in the language the patient wrote in, and doctors go home with every note signed. wai drafts the replies and the notes, a person at the practice approves or signs each one, and the records stay in South Africa under POPIA.

The private care market in Cape Town
Census 2022 counted 4,772,846 people in the City of Cape Town, up from 3,740,031 in 2011, at 1,956 residents per square kilometre.1 About 5.7% of them were born outside South Africa.1
Private care reaches further here than elsewhere. In 2024, 27.5% of Cape Town residents belonged to a medical aid scheme, compared with 15.5% nationally, and use of public facilities as a first port of call was lowest in the Western Cape, at 54.8% of households.2 An estimated 854,000 Western Cape households said they would go first to a private doctor or specialist.2
The rules a practice in Cape Town works under
Doctors register with the Health Professions Council of South Africa. Its Booklet 20, the ethical guidelines on the use of AI dated September 2025, says patients must be informed when a diagnosis or treatment recommendation was determined or assisted by AI, and that a patient who declines AI may not be disadvantaged or refused care.5 It calls it contrary to good practice for AI to be the final decision maker, because only a registered person may diagnose, treat or advise.5 wai drafts and the doctor decides and signs.
The Protection of Personal Information Act 4 of 2013 treats health information as special personal information. Section 26 prohibits processing it, and section 32 lifts that for medical professionals and healthcare institutions when it is necessary for proper treatment and care or for administering the practice.3 Section 22 requires the practice to notify the Information Regulator and the patient as soon as reasonably possible after a security compromise.3
Section 72 limits transfers to a third party in a foreign country to listed grounds, such as a recipient bound by substantially similar protection or the patient's consent.3 The Information Regulator monitors and enforces POPIA for public and private bodies, and a practice's Information Officer must be registered with it before taking up the role.4
wai hosts each clinic's records in its own country, under that country's rules. We set this up with your clinic. wai is compliant with these rules and built for clinics in Cape Town. For a Cape Town practice that means South Africa, and every message, draft, approval and signature sits in an event log the practice can export.
The languages Cape Town patients write in
In the Western Cape, Census 2022 found Afrikaans the language most often spoken at home by 2,954,135 people, or 41.2%, down from 49.8% in 2011. isiXhosa rose to 31.4% from 24.6%, and English to 22.0% from 20.3%.1 Shona, listed separately for the first time, had 141,860 speakers, or 2.0%.1
wai works in any language, and every draft goes to reception for approval before it sends.7 Bring messages from your own patients and judge the drafts.
What a demo shows in Cape Town
A WhatsApp message from Khayelitsha arrives at 8pm asking whether a child's rash needs a visit. wai flags it for the doctor, reads the doctors' busy times from Google Calendar, Outlook or Apple Calendar and drafts a reply with the first open slot, and reception approves it at half past seven. A missed call to the Claremont rooms during a procedure becomes a drafted text in the same thread.
The patient answers the intake questions by voice in any language, and the doctor reads them in English before the visit. Recording consent is taken at booking. In the consult wai drafts the note and the letter, the doctor signs both, and the invoice and the medical aid claim are prepared from the signed note for the team to check.
For health systems, governments and insurers in Cape Town
The Western Cape Department of Health and Wellness, hospital groups and medical schemes each process health information under their own POPIA grounds, and section 32 names medical schemes and their administrators directly.63 For a Western Cape hospital group or medical scheme, wai builds a single pathway first with the organisation's own teams, and the contract fixes who owns the data, the configuration and the model. See wai for health systems and governments.
Where to start
First, add the AI line to the practice's consent wording in Afrikaans, isiXhosa and English, with the Booklet 20 assurance that declining AI will not affect their care, using the consent form template. Second, confirm the Information Officer is registered with the Regulator.
Third, get hosting in South Africa and every subprocessor in writing, and write the section 22 breach routine. Fourth, tally a month of Cape Town enquiries by language and channel. Book a Cape Town demo and bring that count.
Bring a week of Cape Town enquiries, in every language they arrived in, and see the drafts your reception would approve.
