For clinics, centres and hospitals in Malaysia, Kementerian Kesihatan Malaysia and MSQH

MSQH accreditation software, with wai running the whole clinic.

wai runs booking, clinical notes, patient messaging, payments and records for Malaysian clinics, centres and hospitals, and keeps each of them as evidence an MSQH surveyor or a Ministry of Health inspector can read. Patients are answered in the language they write in, see the fee before they commit and leave with a signed note behind them, which is the standard MSQH accreditation rewards.

Updated . Every MOH and MSQH fact is checked against msqh.com.my and the other official pages cited below.

What an MSQH survey day looks like with wai

Malaysia has 11,706 registered private medical clinics and 213 licensed private hospitals, and the ones patients recommend run the same way every day. With wai, an enquiry is answered within the hour in the patient's own language, the fee is shown before the patient commits, the note is signed during the visit, and a complaint sits on the patient's own thread with its response time. An MSQH surveyor can be shown any of it in minutes.

What do the MSQH Medical Clinic Standards ask of a clinic in 2026?

What evidence do MSQH surveyors look at?

Surveyors validate the clinic's own self-assessment on site, through interviews, observation and a random review of documents.2

  1. 01

    Self-assessment

The Survey Questionnaires the clinic completes against every standard, sent to MSQH before the survey.

  2. 02

    Team interviews

Each person in charge of an area interviewed, and staff asked whether practice matches the policy.

  3. 03

    Observation

Surveyors watch the actual workflow without interfering with clinic work.

  4. 04

    Documents

A random review of the documents that support each rating, from minutes to policies and quality projects.

  5. 05

    Records

A sample of health records and personnel records, where the law permits.

  6. 06

    Patients

Client interviews, patient surveys, compliments and complaints with their follow-up.

How is an MSQH accreditation decided?

Every criterion and every standard is rated 1 to 4, then two councillors score the anonymised report from 1 to 30.3

  1. Four-year accreditationSubstantial compliance, a rating of 3 or 4 in all six standards
  2. Delayed accreditationMost standards met, a compliance report within six months
  3. Non-accreditationA significant number not met, wait at least twelve months
  1. 4 years

is the longest MSQH award, with progress reports at 18 and 30 months for a clinic.3

  2. 3 or 4

is the rating every core criterion needs for its standard to rate 3 or 4.3

  3. 12 months

of full operation, and MSQH membership, before a clinic can be surveyed.2

  • A criterion rated 1 or 2 needs a risk assessment, by the clinic in self-assessment and by the surveyors on site.
  • A standard whose overall risk is Critical or High is rated 2 or 1.
  • The Chief Surveyor sends ratings and the team's recommendation to MSQH within one week, and the report within seven days.
  • Two councillors score the anonymised report, 20 to 30 for four-year accreditation and 10 to 19 for delayed accreditation.

What is changing in 2026?

Act 586 and its 2006 Regulations still govern registration and licensing, while the hospital standards and the Ministry's licensing guidance are new this year.

  1. In force today. Act 586 and its 2006 Regulations, P.U.(A) 137/2006 for clinics and P.U.(A) 138/2006 for hospitals and other licensed facilities, in force since 1 May 2006.
  2. New in 2026. MSQH has published its 7th Edition Hospital Accreditation Standards 2026, version 1.1, with a new standard on the governance, ethical use and monitoring of artificial intelligence in patient care. The survey process guide on its site is still the 6th edition guide, so ask MSQH which edition your survey will use.4
  3. Updated guidance. CKAPS reissued its licensing guideline on 6 April 2026, and licence renewals are made online through myBRP@CKAPS.
  4. For clinics. The MSQH Medical Clinic Standards, 2nd edition, approved for implementation in March 2024, are the standards clinics are surveyed against.
  5. MSQH holds ISQua accreditation for its organisation and surveyor training programme from 2024 until 2028, and for its standards from 2022 until 2026, and is seeking re-certification.

Registered or licensed, what Act 586 asks before you open

A private clinic is registered. A private hospital and every other section 3 facility is approved and then licensed, and the licence is renewed every two years.

  1. A private medical or dental clinic needs one application, Borang A on the myMedPCs system, and may operate only once it holds its certificate of registration. Registration is not renewed.5
  2. A private hospital, ambulatory care centre, haemodialysis centre, nursing home, maternity home, blood bank, hospice or community mental health centre needs two stages, approval to establish or maintain and then a licence to operate.
  3. Operating an unregistered clinic or unlicensed facility is an offence under section 5, with a fine of up to RM300,000 or six years in prison for an individual, and up to RM500,000 for a company.
  4. Accreditation is separate. MSQH surveys only licensed services, and a private facility must attach its valid Ministry of Health licence to its survey application.
FacilityUnder Act 586HowHow long it lasts
Private medical or dental clinicRegistration, section 4Borang A on myMedPCs, a no-objection letter to renovate, then the certificate of registrationNot renewed. Changes, transfers and disposal need their own application

The MSQH 7th Edition Hospital Accreditation Standards 2026

Seven hospital-wide service standards apply to every hospital, and 44 more service standards apply to the services it runs.

  1. The hospital-wide standards are 01 Governance, Leadership and Direction, 02 Environmental and Safety Services, 03 Facility and Biomedical Equipment Management and Safety, 04 Nursing Services, 05 Prevention and Control of Infection, 06 Patient and Family Rights, and 07 Health Information Management System, Digital Care and Artificial Intelligence.4
  2. Each standard is organised under the same six areas, Organisation and Management, Human Resource Development and Management, Policies and Procedures, Facilities and Equipment, Safety and Performance Improvement Activities, and Special Requirements. Special Requirements cover safety that cannot be compromised, and a hospital that misses them is not accredited.
  3. Standard 7.6.3 asks for an inventory of every AI tool in use, an AI Tools Register, staff consultation and training before deployment, defined accountability for AI-assisted decisions with human oversight, and telling patients when AI is used in their care.
  4. For an AI scribe, the record of that is a note the clinician reads, edits and signs, the transcript beside it, and the patient's consent. wai keeps all three on the record.

Questions

What is the best MSQH accreditation software?

The system your clinic already runs on, so the evidence is recorded as the work is done. Look for one record behind booking, notes, messaging, payments and access, with signed contemporaneous notes and a full audit trail. wai is built that way, to help you achieve MSQH accreditation.

Is there MSQH or Ministry of Health approved software?

The Malaysian Society for Quality in Health accredits hospitals, medical clinics, dental clinics and dialysis facilities against its own standards. It does not approve, certify or endorse software, wai included, and software cannot earn an accreditation on a provider's behalf. wai keeps the records your team uses to show MSQH how care is delivered, and MSQH decides the accreditation. The Ministry of Health registers and licenses facilities under Act 586, and does not endorse software either.

Does a private clinic in Malaysia need a licence or a registration?

A registration. A private medical or dental clinic is registered under section 4 of Act 586, and the registration is not renewed. Private hospitals, ambulatory care centres, haemodialysis centres and the other section 3 facilities need approval and then a licence, renewed every two years.

Is MSQH accreditation compulsory?

No, it is voluntary. The Ministry recognises MSQH as the national accreditation body for healthcare facilities and services, and a private facility needs its Ministry licence before MSQH will survey it.

How long does MSQH accreditation last?

Four years at most, subject to keeping the standards. A clinic with four-year accreditation sends progress reports at 18 and 30 months, and MSQH sends the renewal form six months before it expires.

Does MSQH ask about AI?

The 7th Edition Hospital Accreditation Standards 2026 do. Standard 7.6.3 asks for an inventory of AI tools, accountability for AI-assisted decisions with human oversight, and patients told when AI is used in their care.

Does wai work for hospitals and multi-site groups?

Yes. Every location runs on the same record, inbox and reports. Larger organisations usually start with one pathway, prove it on their own data, and widen it from there.

Where does wai keep our records?

wai hosts each clinic's records in its own country, under that country's rules. We set this up with you before go-live. Encrypted, with a full audit log.
