Key points
- A booking runs through five steps, from request to reminder, and a reschedule runs the same steps again on the same record.
- In a Mayo Clinic study, staff-scheduled well-child visits needed more than one scheduling step 28.32% of the time, against 6.93% when parents booked themselves.1
- 29.5% of those self-scheduling actions happened outside regular business hours, so booking has to keep working after the desk closes.1
- Each clinician's busy times are read from Google Calendar, Outlook or Apple Calendar, so patients are only offered times that are open.3
- wai holds the new slot and drafts the reply, and a person at the clinic approves it before it sends.3
Summary
Run booking and rescheduling as one workflow on one patient record. Offer only times when the clinician is available, hold the slot while the patient confirms, and send a confirmation with one press to move it. A person at the clinic approves each reply before it sends.
A full diary, with every booking confirmed while the patient is still on the phone or in the WhatsApp thread, and every change settled just as fast. The work behind it is one continuous workflow on one patient record, where a slot offered is a slot held.

| Hold | Slot reserved for one patient | Hold window by channel, then automatic release | Double bookings and expired holds |
| Confirm | Reply approved by a person and confirmation sent | Confirmation carries the terms and a move link | Share of bookings confirmed in one step |

The five steps of every booking
Whatever the channel, a booking moves from request to matched availability, then to a held slot, a confirmation and a reminder. A reschedule runs the same steps again from a booking that already exists.
Aim for fewer steps per finished booking. Over 13 months of well-child visits at Mayo Clinic, staff-scheduled appointments needed more than one action, such as a schedule followed by a cancel and a reschedule, 28.32% of the time, against 6.93% for appointments parents booked themselves.1 Exclusive self-schedulers finalised 93.1% of their appointments in a single step.1
Offering only times that are open
A consultation may need the doctor, a room and a partner provider such as an imaging centre, so offer a time only when each of them is available.
Each clinician's busy times are read from Google Calendar, Outlook or Apple Calendar. A doctor who blocks Thursday afternoon in their own calendar removes those times from booking, and patients only ever see open times.3
Lead with the earliest matching time and give two alternatives, so the diary fills from the front.
Holding a slot while the patient decides
A hold reserves one slot for one patient for a short, stated time while they confirm or pay, so each time is offered to one patient at once.
Set the hold window by channel. A live call needs a few minutes, and a WhatsApp or email reply sent after hours may need until the next morning. When the window ends, release the slot and offer the next best time.
Where a deposit applies, the hold and the payment link travel together, and payment confirms the booking. The mechanics are in our guide to deposits and payment links, and the wording patients agree to is in our guide to cancellation policies and deposit terms.
Rescheduling as one reply
Answer a request to move with a new time already held. The patient writes that Thursday no longer works, and the reply offers Friday at 10:00 or Monday at 14:30, with the original slot kept until they choose.
In a US survey of 2,338 consumers in late 2018, 40% of millennials against 19% of baby boomers said they would choose a provider that lets them book, change or cancel online.2 Put one press to move the appointment in every confirmation, so a change of plans ends in a moved slot.4
Once the new time is confirmed, release the old slot to the waitlist straight away.
A confirmation that answers the next question
Say where to come, what to bring and the cancellation terms, include the receipt, and offer one press to add the visit to a calendar and one to move it.4 Send it in the clinic's name, so a reply reaches a person.
Intake questions sent with the confirmation come back before the visit, and the clinician opens the consultation with the history in hand. In the Mayo Clinic study, no-shows were 3.07% for self-scheduled visits against 4.12% for staff-scheduled ones, a difference that was not significant.1
How wai runs the workflow
wai Admin keeps each patient's WhatsApp, email, web and phone messages in one thread on one record. When a patient asks to move Thursday, the new slot is held and the reply is drafted, and a person at the clinic approves it before it sends.3 Availability is checked across the clinic's doctors, the patient and any partner providers, and a slot freed by a change is backfilled from the waitlist.3
Bookings on the clinic's own website follow the same path through wai Checkout, with the earliest time shown first and three taps to book and pay.4 Each clinician's busy times come from their Google Calendar, Outlook or Apple Calendar, and booked video calls are added to the clinic's Google Calendar.
Where to start
Map one week of real bookings against the five steps. For each, note the channel, the number of messages it took and how long the slot was held.
Then set two rules, a hold window for each channel and the release of every moved slot to the waitlist. After four weeks, measure messages per booking, the share of bookings needing more than one step and slots refilled after a change. Our guide to adding online booking to a clinic website covers the patient-facing half.
Bring a week of your own booking requests. We show each one held and confirmed, with your team approving every reply.
Frequently asked questions
How long should a clinic hold a slot for a patient?
Long enough for the patient to answer on the channel they used, a few minutes on a live call and until the next morning for a message sent after hours. Release the slot when the window ends and offer the next time.
How does booking know when a clinician is available?
It reads each clinician's busy times from Google Calendar, Outlook or Apple Calendar, so a block in their own calendar removes those times from booking and patients are only offered open times.3 Booked video calls are added to the clinic's Google Calendar.
Can patients reschedule without calling the clinic?
Yes. Every confirmation carries one press to move the appointment.4 A request by message is answered with a new time already held, and the team approves the reply.
Does letting patients book themselves increase no-shows?
In the Mayo Clinic well-child study, no-shows were 3.07% for self-scheduled visits against 4.12% for staff-scheduled visits, a difference that was not significant.1 Send reminders for every booking either way.
How do we book a visit that needs a doctor and a partner provider?
Treat each as availability that must agree before a time is offered. wai checks availability across the clinic's doctors, the patient and any partner providers.3
Does the AI book or move appointments on its own?
wai holds the slot and drafts the reply, and a person at the clinic approves it before it sends.3
