Key points

Summary

A front desk AI should recognise any clinical message, flag it and pass the full conversation and the patient's record to a named clinician, who replies in a message they sign. Emergency signs trigger wording a clinician has approved in advance, sent by the person on duty, which directs the patient to their local emergency number while the duty clinician is alerted.

A patient who writes about a worrying symptom hears from a doctor within minutes, and that doctor already has the whole conversation. The front desk AI makes that possible by spotting the message that needs a clinician and putting it in front of one.

Recognise and hand over, never assess

Most messages to a clinic are about bookings, prices, directions and payments. Some are clinical, such as a symptom, a result, a medication question or a worry after treatment. The AI recognises the clinical message and hands it to a person, and the clinician does the assessing and advising.

A good reception desk already works this way, passing a call to a nurse or doctor the moment a symptom comes up. The wider escalation map, support routes and accessibility are in our guide to clinic AI support and escalation. This page covers the handover itself.

Triage belongs with clinicians

In a 2015 BMJ audit, 23 publicly available online symptom checkers were tested with 45 standardised patient vignettes and gave appropriate triage advice in 57% of 532 evaluations.1 They advised emergency care in 80% of emergency cases and gave appropriate advice in 33% of cases where self care was reasonable, and the authors described their advice as generally risk averse.1

Spotting that a message is clinical is a far easier task than judging how urgent it is. Give the software the first task and the clinician the second.

What regulators look at

MHRA guidance on stand-alone software lists triage signposting among the outputs of symptom checkers. Software that only signposts the user to suitable care is unlikely to be a medical device, while software that filters by red flag, severity or likelihood of a match may be one.2

In the US, FDA's clinical decision support guidance, updated in January 2026, sets out which decision support functions fall outside the definition of a device, and states that existing digital health policies continue to apply to device software functions intended for patients or caregivers.3

A front desk that signposts and hands over keeps clinical judgement with registered clinicians. Check how your own setup is classed in each country you operate in.

Red flags and emergency wording

The clinical lead writes the list of words and situations that move a thread to a clinician at once, and reviews it monthly. Most lists include chest pain, difficulty breathing, heavy bleeding, sudden weakness or confusion, a severe reaction after treatment, thoughts of self harm and any mention of an unwell child.

Emergency wording is written by a clinician and approved once. It tells the patient to call their local emergency number now and says the clinic team has been alerted. The person on duty sends it at once, and any further advice comes from a clinician.

Set the number for every country the clinic's patients write from. Across the EU, 112 reaches an ambulance, the fire brigade or the police from any fixed or mobile phone, free of charge.4

A handover with the whole conversation

The handover carries the full thread across every channel, the patient's record, the reason the message was flagged and any holding reply already sent, so the patient never repeats themselves.

wai Admin reads each enquiry for intent and tone and loops in a doctor the moment anything turns clinical, with the full story attached.5 The front desk approves a short holding reply so the patient knows a clinician has the message, and the clinician answers in a reply they sign. The wai security page puts the boundary in writing, with clinical replies needing a clinician's signature and operational replies needing the team's approval.6

Every message, draft, approval and signature goes into an event log the clinic can export, showing how long each clinical thread waited and who answered it.6

Where to start

Write the red flag list and the emergency wording with your clinical lead, including the local emergency number for every country your patients write from, and name a duty clinician for every hour the inbox is open, evenings included.

Then test the handover. Send ten clinical messages through every channel and check that each reached the duty clinician with the full thread, a holding reply approved and the time recorded. Repeat monthly and track the time from message to clinician pickup.

Bring ten real clinical messages from your inbox. We show each one flagged, handed to a doctor with the full thread and logged.

Frequently asked questions

Does the AI give patients medical advice?

The AI recognises clinical messages and hands them to a clinician, who answers in a reply they sign.6

What happens if a patient describes an emergency at night?

The thread is flagged and the duty clinician is alerted at once. The person on duty sends the clinic's approved emergency wording, which directs the patient to their local emergency number.

Is a front desk AI a medical device?

That depends on what it does. MHRA guidance says software that only signposts users to suitable care is unlikely to be a device, while software that filters by red flag or likelihood may be one.2 Keep triage with clinicians and check your own country's rules.

What does the clinician see when a message is handed over?

The full conversation across every channel, the patient's record and the reason it was flagged.5

Who writes the red flag list?

The clinic's clinical lead, reviewed monthly against the threads that escalated and any that should have.

How do we know escalation is working?

Review the event log weekly.6 Check the time from message to clinician pickup, and look for any clinical thread the front desk answered without a clinician.
