Nothing here is taken from a review site, a directory or a competitor's comparison page, because those are where stale numbers live. Where a vendor publishes nothing, the entry says so rather than repeating a figure somebody else guessed.
Three of the twelve products publish no price at all. Semble, Pabau and iaptus each require a conversation before a buyer learns what they cost. Two more publish only in United States dollars while selling into the United Kingdom.

Who publishes and who does not
Nine of the twelve publish a figure a buyer can act on. Jump publishes the most, with seven volume bands plus migration, training, card and SMS costs. WriteUpp publishes a full ladder in pounds including its add-on prices. Carebit publishes three tiers and the cap on the entry one.
Semble, Pabau and iaptus publish nothing. That is a legitimate way to sell and it is also a slower way to buy, because a clinic owner comparing on a Sunday evening cannot rule any of the three in or out without starting a sales process.
The pattern is worth naming. The larger the installed base, the less likely the price is published. That is a commercial choice about who controls the conversation rather than a fact about the software.
The caps on entry plans
Two entry tiers here are capped by volume rather than by feature. Carebit's Starter at £79 a clinician is limited to 18 bookings a month, and Jane's Balance at $54 is limited to 20 appointments a month. Both are published, both are read on 8 September 2026, and both are easy to pass without noticing.
The practical rule is to price the tier you will be on in six months. A clinician seeing 5 patients a week clears either cap inside a month, and the step up is a different number entirely. Carebit's next tier is £119 a clinician paid yearly in advance or £149 monthly.
Currency, which the tables usually hide
Cliniko and Medesk publish only in United States dollars while selling to United Kingdom clinics, on their own pages and on the same date. Jane and SimplePractice publish in dollars for a North American market, which is straightforward.
For a British buyer this means two of the cheapest options on the list carry an exchange rate on the software line. It is a small amount of money at these figures and it is the kind of detail that makes a spreadsheet comparison wrong by a few percent.
What this table cannot tell you
Every product here is priced as software, which means the price is for a place to do the work rather than for the work. Two clinics on the same tier can have completely different costs once you count the people needed to operate it.
That is the reason a licence comparison usually settles nothing. The larger number in most private clinics is the reception and administrative wage, and no line in this table moves it. wai is priced above this market and published on one page, because it covers the enquiry, the booking, the note and the letter rather than providing somewhere to record them.

Method
One reader, one day, each vendor's own pricing page. Where a page returned an error or carried no figure, the entry records that rather than substituting a third party number. Prices exclude tax unless the vendor stated otherwise, and where a vendor states a currency it is reproduced as published.
Four products were considered and left out because no published figure could be verified on the date. Athenahealth's pricing page returned an access error, DrChrono showed tiers with a quote request and no figures, MedicalDirector had no pricing page, and OSCAR is open source with per provider hosting quoted by each service provider rather than centrally.

Which private practice EHR is cheapest?
On published entry price, WriteUpp at £19.95 a month in the United Kingdom and Halaxy which gives the core platform away in Australia. Medesk runs a free tier covering 50 appointments a month. Cheapest entry is rarely cheapest in use, because add-ons, caps and payment fees move the real figure, so model the tier you will be on in six months.
Which practice management systems do not publish prices?
Of the twelve read on 8 September 2026, Semble, Pabau and iaptus published no figure on their own sites. Athenahealth, DrChrono and MedicalDirector also published nothing verifiable on that date. Why do some vendors hide their pricing?
Usually because the price varies by user count, country and add-ons, and because a sales conversation converts better than a page. It is a legitimate commercial choice. It also slows the buyer down, which is the point, and it means every third party figure circulating for those products is an estimate rather than a published fact.
How often is this index updated?
Every figure carries the date it was read, and this index is refreshed with a substantive check rather than a re-stamped date. Prices in this category move several times a year, so treat any line older than a quarter as indicative and check the vendor's own page before you rely on it.
